静脉内平滑肌瘤的临床特征及诊疗进展

摘要
静脉内平滑肌瘤(intravascular leiomyoma，IVL)是一种罕见的子宫肌瘤亚型，其特征表现为组织学良性的平滑肌瘤在子宫和宫外静脉系统内侵袭性生长，并可侵入下腔静脉或右心房，甚至延伸至右肺动脉，引起肺动脉栓塞，可在短时间内对患者造成生命威胁。IVL的临床表现及术前影像学缺乏特异性，术前诊断几乎不可能。近年来，该病发生率有上升的趋势，然而对于其病因、临床表现、术前诊断、治疗方式、预后等问题一直存在广泛争议。

Abstract
Intravascular leiomyoma (IVL) is a rare subtype of uterine leiomyomata. Characterized by histologically benign leiomyomas in the uterine and extrauterine venous system was invasive growth, tumors could invade the inferior vena cava or reach the right atrium, and even extends to the right pulmonary artery, causing pulmonary embolism, and therefore, threats to life in a short period of time. Clinical manifestations and imaging of IVL are lack of specificity, and preoperative diagnosis is almost impossible. In recent years, the incidence of this disease has been on the rise. However, there has been widespread controversy over its causes, clinical manifestations, preoperative diagnosis, treatment methods, and prognosis.
