专项护理干预在晚期癌症姑息治疗患者家属上的临床应用

摘要
目的:探讨专项护理干预在晚期癌症姑息治疗患者家属上的应用效果。方法:选择自2014年3月至2015年6月在我院治疗的临床上被确诊为癌症晚期且接受姑息治疗的101例患者及其家属101例作为研究对象,按照入院治疗的先后顺序进行分组。其中对照组患者共52例,仅给对照组患者进行姑息治疗;观察组患者共计49例,在对照组姑息治疗的基础上同时对其家属进行专项护理干预。观察分析两组患者家属的焦虑自评量表(Self-Rating Anxiety Scale,SAS)、抑郁自评量表(Self-Rating Depression Scale,SDS)评分,并对两组患者在干预前后的焦虑及郁闷指数情况和两组患者在干预后的QLQ-C30量表中各项功能得分情况进行分析对比。结果:观察组家属的SDS、SAS两项评分均显著低于对照组,差异比较有统计学意义(P<0.05)。对比两组患者干预前后的焦虑、郁闷指数,干预前,两组患者无明显差异(P>0.05);干预后,观察组的指数显著低于对照组,对比具有统计学意义(P<0.05)。观察组患者的QLQ-C30各项功能得分均显著高于对照组,差异显著,具有统计学意义(P<0.05)。结论:针对癌症晚期姑息治疗患者家属的专项护理干预效果显著,不仅对患者家属的焦虑和抑郁有缓解,更能够影响患者的情绪并提高患者的生活质量,值得在临床上推广应用。

The clinical application of special nursing intervention in families of patients with advanced cancer palliative care

Abstract
Objective: To research the clinical application effects of special nursing intervention in families of patients with advanced cancer palliative care. Methods: From March 2014 to June 2015, 101 cases with advanced cancer and 101 cases of family members were selected as the research objects. According to the order of treatment, they were divided into two groups. The control group had 52 cases, they were treated with palliative care, and the observation group had 49 cases, on the basis of palliative care in the control group, the special nursing intervention was carried out to the families of the patients in the observation group. Observed and analyzed the Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS) score in two groups of patients. Analyzed and compared anxiety and depression index before and after the intervention, compared the functional score of the QLQ-C30 scale after the intervention in two groups. Results: The SAS, SDS score of the families in the observation group were significantly lower than those in the control group (P<0.05). Compared the anxiety and depression index of the two groups of patients before and after the intervention, there is no significant difference between the two groups of patients before the intervention (P>0.05); but the index in the observation group was significantly lower than that in the control group after the intervention (P<0.05). The functional score of each item in the QLQ-C30 scale in the observation group was significantly higher than that in the control group (P<0.05). Conclusion: The effect of the special nursing intervention for patients’ families with advanced palliative care in the treatment of patients with advanced cancer is significant. The special nursing intervention for patients’ families can not only alleviate the anxiety and depression of patients’ families, but also affect the patients’ mood and improve the quality of life. It is worthy of clinical application.
