局部晚期鼻咽癌洛铂单药与PF方案同步放化疗临床观察

摘要
目的:比较洛铂单药用于局部晚期鼻咽癌(nasopharyngeal carcinoma,NPC)同步放化疗与顺铂(cisplatin,CDDP)、5-氟尿嘧啶(fluorouracil,5-FU)联合方案同步放化疗的疗效及毒副反应。方法:101例局部晚期NPC分为两组,49例接受CDDP+5-FU联合方案同步放化疗(PF组),52例接受洛铂单药同步放化疗(L组),比较近期疗效、5年生存率及放疗期间毒副反应。结果:PF、L两组患者放疗一月后客观有效率分别为95.9%、96.2%(P=0.889),5年生存率分别为53.1%、55.8%(P=0.921),毒副反应恶心呕吐和口腔黏膜反应PF组均较L组明显(P<0.05)。结论:洛铂单药用于局部晚期NPC同步放化疗与CDDP+5-FU联合方案同步放化疗的疗效近似,总生存率无统计学差异,洛铂单药组的恶心呕吐、口腔黏膜反应较轻。

Comparison of the efficacy and adverse effects between cisplatin combination with 5-FU and lobaplatin alone plus concurrent radiotherapy for local advanced nasopharyngeal carcinoma

Abstract
Object: To investigate the efficacy and adverse effects between lobaplatin alone and cisplatin (CDDP) combination with fluorouracil (5-FU) plus concurrent radiotherapy for local-regionally advanced nasopharyngeal carcinoma. Methods: A total of 101 patients with local-regionally advanced nasopharyngeal carcinoma were divided into two groups, 49 patients received CDDP combination with 5-FU and 52 patients were given lobaplatin alone (group L), respectively, plus concurrent radiotherapy. Compared of the two groups with short-term effects, 5 years survival rate and adverse effects during radiotherapy. Results: A month after radiotherapy, objective response rate was 95.9% for group PF and 96.2% for group L (P=0.889), and 5 years survival rate was 53.1% vs. 55.8% (P=0.921), respectively. Adverse effects, including nausea, vomiting and oral mucosa reaction, in group PF were more obvious than group L (P<0.05). Conclusion: The efficacy between lobaplatin alone and CDDP combination with 5-FU plus concurrent radiotherapy for local-regionally advanced nasopharyngeal carcinoma was similar. Total survival rate between two groups had no significant difference. Nausea, vomiting and oral mucosa reaction in group L were significantly lower than group PF.
