积极反刍思维训练对慢性肾脏病5期血液透析患者负性情绪、疲乏状况和睡眠质量的影响

摘要

目的:探讨积极反刍思维训练对慢性肾脏病(chronic kidney disease,CKD)5期血液透析患者负性情绪、疲乏状况和睡眠质量的影响。方法:选取2020年1月至2021年6月江苏省人民医院治疗的CKD5期血液透析患者100例,随机分为对照组(n=50)与观察组(n=50)。对照组予以常规护理,观察组实施积极反刍思维训练;于干预前后应用焦虑自评量表(Self-Rating Anxiety Scale,SAS)、抑郁自评量表(Self-Rating Depression Scale,SDS)评价患者的焦虑、抑郁情绪,应用疲劳量表(Fatigue Scale-14,FS-14)评定患者的疲乏程度,应用匹兹堡睡眠指数量表(Pittsburgh Sleep Quality Index,PSQI)评定患者的睡眠质量。结果:干预后,观察组SAS评分、SDS评分均显著低于对照组,差异均有统计学意义(均P<0.05);干预后,观察组FS-14量表总分及身体疲乏、精神疲乏维度评分均显著低于对照组(均P<0.05);干预后,观察组PSQI评分显著低于对照组(P<0.05)。结论:积极反刍思维训练不仅可缓解CKD5期血液透析患者情绪状态及疲乏状态,还有助于改善其睡眠质量,是促进患者身心健康及提高其生活质量的有效干预手段。

Effect of positive rumination training on negative emotion, fatigue, and sleep quality in hemodialysis patients with chronic kidney disease stage 5

Abstract

Objective: To explore the effect of positive ruminat thinking training on negative emotions, fatigue status and sleep quality of chronic kidney disease stage 5 (CKD5) hemodialysis patients. Methods: A total of 100 hemodialysis patients with CKD5 admitted to our hospital from January 2020 to June 2021 were selected and randomly divided into a control group (n=50) and an observation group (n=50). The control group received routine nursing and the observation group received positive ruminat thinking training. The Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) were used to evaluate the anxiety and depression of patients before and after the intervention. The Fatigue Scale-14 (FS-14) was used to evaluate the fatigue degree of patients before and after the intervention. And the Pittsburgh Sleep Quality Index (PSQI) was used to evaluate the sleep quality of patients before and after the intervention. Results: After the intervention, the SAS score and SDS score of the observation group were significantly lower than those of the control group, and the difference was statistically significant (both P<0.05). After the intervention, the total score of FS-14 scale, physical fatigue, and mental fatigue scores in the observation group were significantly lower than those in the control group (all P<0.05). After the intervention, the PSQI score of the observation group was significantly lower than that of the control group (P<0.05). Conclusion: Positive ruminat thinking training can not only alleviate the emotional state and fatigue state of hemodialysis patients with CKD5, but also help to improve their sleep quality, which is an effective intervention method to improve the physical and mental health and quality of life of patients.
