摘要

目的:探讨超声内镜引导下细针穿刺活检(endoscopic ultrasound-guided fine-needle aspiration,EUS-FNA)对胰腺占位性病变的诊断价值。方法:选取2016年12月至2019年11月45例在南通市肿瘤医院内镜中心诊断为胰腺占位性病变的患者,在超声内镜引导下行细针穿刺活检,分析穿刺病理结果,并与最终诊断结果进行比较。结果:EUS-FNA诊断准确率为80.00%,与最终诊断结果进行比较差异无统计学意义。EUS-FNA诊断恶性胰腺占位的敏感度、特异度、阳性预测值分别为85.71%、100%、100%。术后并发症发生率为4.44%。无出血、穿孔、重症胰腺炎等严重并发症。结论:EUS-FNA对胰腺占位性病变的早期诊断价值较高,且安全性好。

Diagnostic value of endoscopic ultrasound-guided fine-needle aspiration in space-occupying lesions of the pancreas

Abstract

Objective: To investigate the diagnostic value of endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) for pancreatic lesions. Methods: Forty-five patients diagnosed with pancreatic space occupying lesions in Endoscopy Center of Nantong Tumor Hospital from December 2016 to November 2019 were enrolled. Fine-needle aspiration biopsy was performed under the guidance of endoscopic ultrasonography. The EUS-FNA histological results were compared with the final clinical diagnosis. Results: The sensitivity, specificity and positive predictive values of EUS-FNA for diagnosing pancreatic lessons were 85.71%, 100% and 100%, respectively. The diagnostic accuracy of EUS-FNA was 80%. There was no statistical significance in diagnostic accuracy between the EUS-FNA dignosis and the final diagnosis. The incidence of postoperative complications was 4.44%. There were no serious complications associated with EUS-FNA, such as bleeding, perforation and severe pancreatitis. Conclusion: EUS-FNA is a safe and effective method for the diagnosis of pancreatic space-occupying lesions.
