摘要

目的:探讨悬吊运动训练(sling exercise training,SET)对痉挛型脑瘫患儿的康复治疗效果。方法:选取2017年10月至2020年6月海南省妇女儿童医学中心儿童康复科和儿科收治的104例痉挛型脑瘫患儿,区组随机法分成对照组与SET组,每组各52例。2组均给予常规康复训练,SET组同时给予SET,6个月后采用改良Ashworth量表(Modified Ashworth Scale,MAS)、Berg平衡量表(Berg Balance Scale,BBS)和粗大运动功能评估量表(Gross Motor Function Measure Scale-88,GMFM-88)评估康复效果。结果:与同组治疗前比较,2组治疗6个月后内收肌、腓肠肌和比目鱼肌的MAS评分均明显下降,BBS评分、GMFM-88总分及D、E功能区评分均明显提高,差异均有统计学意义(均P<0.05);与对照组比较,SET组治疗6个月后内收肌、腓肠肌和比目鱼肌的MAS评分明显较低,BBS评分、GMFM-88总分及D、E功能区评分均显著较高,差异均有统计学意义(均P<0.05)。结论:SET能明显降低痉挛型脑瘫患儿下肢肌张力,改善躯体平衡能力和肢体运动功能,康复治疗效果显著。

Effect of sling exercise training on rehabilitation of children with spastic cerebral palsy

Abstract

Objective: To investigate the rehabilitation effect of sling exercise training (SET) on children with spastic cerebral palsy. Methods: From October 2017 to June 2020, 104 children with spastic cerebral palsy in the Department of Rehabilitation and Pediatrics were randomly divided into a control group and an SET group, 52 cases in each group. Both groups were given routine rehabilitation training, while SET group was given SET at the same time. Six months later, the rehabilitation effect was evaluated by modified Ashworth Scale (MAS), Berg Balance Scale (BBS) and Gross Motor Function Measure Scale-88 (GMFM-88). Results: Compared with the same group before the treatment, the MAS scores of adductors, gastrocnemius, and soleus in the two groups decreased significantly after 6 months of treatment, while the BBS score, GMFM-88 total score, and D, E functional area score increased significantly (P<0.05). Compared with the control group, the MAS scores of adductors muscle, gastrocnemius muscle, and soleus muscle in the SET group were significantly lower, and BBS score, GMFM-88 total score and D, E functional area score were significantly higher. The difference was statistically significant (P<0.05). Conclusion: SET can significantly reduce the lower limb muscle tension and improve the body balance ability and limb motor function in children with spastic cerebral palsy, which has significant rehabilitation effect.
