妊娠期糖尿病miR-29、miR-372-3p、miR-375表达水平及其与胰岛素抵抗的相关性

摘要
目的:探讨妊娠期糖尿病(gestational diabetes mellitus,GDM)miR-29、miR-372-3p、miR-375表达水平及其与胰岛素抵抗(insulin resistance,IR)的相关性。方法:选取2019年5月至2020年5月南京市中西医结合医院诊治的220例GDM患者为研究组,并选取同期220例糖耐量正常的孕妇为对照组。检测并对比两组孕妇空腹血糖(fasting blood glucose,FBG)、空腹胰岛素(fasting insulin,FINS)及糖化血红蛋白(glycosylated hemoglobin,HbAlc)水平,计算胰岛素抵抗指数(HOMA-IR),比较两组孕妇外周血miR-29、miR-372-3p、miR-375的表达水平,并分析其与IR的相关性。结果:研究组FINS水平明显低于对照组,FBG、HbAlc水平及HOMA-IR均明显高于对照组(均P<0.05);研究组外周血miR-29的表达水平明显低于对照组,miR-372-3p、miR-375的表达水平均明显高于对照组(均P<0.05)。Pearson相关性分析结果显示:研究组外周血miR-29的表达水平与HOMA-IR呈负相关(r=−0.379,P<0.05),外周血miR-372-3p、miR-375的表达水平与HOMA-IR呈正相关(均r=0.530、0.628,P<0.05)。结论:GDM患者外周血miR-29、miR-372-3p、miR-375的表达水平跟糖耐量正常的孕妇相比差异具有统计学意义,临床检测其表达水平对诊断GDM有较高的价值,且三者均与患者HOMA-IR有一定相关性,参与IR的发生、发展过程。

Abstract
Objective: To investigate the expression levels of miR-29, miR-372-3p, and miR-375 in gestational diabetes mellitus (GDM) and their correlation with insulin resistance (IR). Methods: A total of 220 GDM patients who were diagnosed and treated in our hospital from May 2019 to May 2020 were selected as a study group, and 220 pregnant women with normal glucose tolerance during the same period were selected as a control group. The fasting blood glucose (FBG), fasting insulin (FINS), and glycosylated hemoglobin (HbAlc) levels in pregnant women of the two groups were detected and compared, and the insulin resistance index (HOMA-IR) was calculated, the expression levels of miR-29, miR-372-3p and miR-375 in peripheral blood of pregnant women of the two groups were compared, and the correlation between them and IR was analyzed. Results: The level of FINS in the study group was significantly lower than that in the control group, and the levels of FBG, HbAlc and HOMA-IR were significantly higher than those in the control group (P<0.05); the expression level of miR-29 in peripheral blood of the study group was significantly lower than that of the control group, and the expression levels of miR-372-3p and miR-375 were significantly higher than those of the control group (P<0.05); the results of Pearson correlation analysis showed that the expression level of miR-29 in peripheral blood of the study group patients was negatively correlated with HOMA-IR (r=−0.379, P<0.05), and the expression levels of miR-372-3p and miR-375 in peripheral blood were positively correlated with HOMA-IR (r=0.530, 0.628, P<0.05). Conclusion: The expression levels of miR-29, miR-372-3p, and miR-375 in the peripheral blood of GDM patients are statistically different from those of pregnant women with normal glucose tolerance, clinical detection of their expression levels is of higher value for the diagnosis of GDM, and all of them are correlated to HOMA-IR of patients to some extent, and are involved in the occurrence and development of IR.
