下肢远端骨折患者超声股四头肌及股动脉血流动力学检测及临床意义

摘要

目的:研究下肢远端骨折患者超声股四头肌及股动脉血流动力学检测水平变化及临床意义。方法:选取四川省骨科医院2019年4月至2020年6月收治的53例下肢远端骨折患者,用彩色多普勒超声诊断仪测定患者入院第1、14天健侧与患侧股四头肌的股直肌横截面积、股内侧肌厚度、股外侧肌厚度、股中间肌厚度、股直肌厚度,股动脉血流动力学包括股动脉舒张末期速度、血流速度、搏动指数、舒张早期反向血流速度及反向血流持续时间及收缩期峰值速度。结果:入院第14天,患者健侧与患侧股直肌横截面积、股内侧肌厚度、股外侧肌厚度、股中间肌厚度、股直肌厚度均较入院第1天明显减小,且患者健侧与患侧比较,差异有统计学意义(P<0.05)。入院第14天,患者健侧与患侧舒张末期速度,平均血流速度均较入院第1天明显上升,搏动指数、舒张早期反向血流速度及反向血流持续时间均较入院第1天明显降低,且患者健侧与患侧比较,差异有统计学意义(P<0.05),入院第14天,患者健侧与患侧收缩期峰值速度与入院第1天比较,差异无统计学意义(P>0.05)。结论:下肢远端骨折卧床制动容易发生废用性肌萎缩,超声表现以股四头肌横截面积及厚度减小,股动脉舒张期血流参数变化为主。

Clinical significance of ultrasonic detection of quadriceps femoris and femoral artery hemodynamics in patients with distal limb fracture

Abstract

Objective: To study the changes and clinical significance of ultrasonic detection of quadriceps femoris and femoral artery hemodynamics in patients with distal limb fracture. Methods: Fifty-three patients with distal lower extremity fractures in Sichuan Provincial Orthopaedic Hospital from April 2019 to June 2020 were selected for the study. The quadriceps femoris (cross-sectional area of rectus femoris, thickness of medial femoris, thickness of lateral femoris, thickness of medial femoris, thickness of rectus femoris), femoral artery hemodynamics (femoral artery end diastolic velocity, mean blood flow velocity, pulsatility index, early diastolic reverse blood flow velocity and reverse blood flow duration, peak systolic velocity) were measured by color Doppler ultrasound diagnostic instrument on the 1st and 14th day after admission. Results: On the 14th day of admission, the cross-sectional area of rectus femoris, the thickness of medial femoris, lateral femoris, medial femoris and rectus femoris of the healthy side and the affected side were significantly decreased compared with the first day of admission, and the difference was statistically significant (P<0.05). On the 14th day of admission, the end diastolic velocity and average blood flow velocity of the healthy side and the affected side were significantly higher than those on the 1st day of admission, while the pulsatility index, early diastolic reverse blood flow velocity and reverse blood flow duration were significantly lower than those on the 1st day of admission, and the difference was statistically significant (P<0.05), there was no significant difference in the peak systolic velocity between the healthy side and the affected side compared with the first day after admission (P>0.05). Conclusion: Patients with distal limb fracture are very likely to develop disuse muscular atrophy due to the immobilization by lying in bed with the main features of ultrasound in the reduction of cross-sectional area and thickness of quadriceps femoris and change of blood flow parameters of femoral artery in diastolic period.
