类风湿关节炎患者血脂代谢特点及与DAS28评分和颈动脉内膜中层厚度的相关性分析

摘要
目的:探讨类风湿性关节炎(rheumatoid arthritis,RA)患者的血脂代谢特点及与28个关节疾病活动度(disease activity score in 28 joints,DAS28)评分和颈动脉内膜中层厚度(carotid intima-media thickness,CIMT)的相关性。方法:选取2016年6月至2020年4月海口市人民医院收治的82例RA患者,记为RA组。另选取同期体检的35例健康志愿者,记为对照组。检测两组总胆固醇(total cholesterol,TC)、三酰甘油(triacylglycerol,TG)、低密度脂蛋白(low-density lipoprotein,LDL-C)、高密度脂蛋白(high-density lipoprotein,HDL-C)、脂蛋白a(lipoprotein a,LPa),采用DAS28评分评价RA组疾病活动度,超声检查CIMT,对RA组血脂水平、DAS28评分和CIMT进行分析。结果:RA组TC、TG、LDL-C、LPa高于对照组,HDL-C低于对照组,血脂异常率68.29%高于对照组的22.86%,差异有统计学意义(P<0.05);RA活动期组LPa高于非活动期组,HDL-C低于非活动期组,差异有统计学意义(P<0.05);RA患者CIMT异常组TC、TG、LDL-C、LPa水平高于CIMT正常组,HDL-C低于CIMT正常组,差异有统计学意义(P<0.05)。RA患者HDL-C与DAS28评分呈负相关,LPa与DAS28评分呈正相关(r=−0.335、0.382,P<0.05),TC、TG、LDL-C、LPa水平与CIMT呈正相关,HDL-C与CIMT呈负相关(r=0.421、0.435、0.418、0.429、−0.437,P<0.05)。结论:RA患者血脂代谢紊乱比较常见,且血脂水平与DAS28评分和CIMT密切相关,临床应重视RA患者血脂监测和管理。

Characteristics of blood lipid metabolism in patients with rheumatoid arthritis and its correlation with DAS28 score and carotid intima-media thickness

Abstract
Objective: To investigate the characteristics of blood lipid metabolism in patients with rheumatoid arthritis (RA) and its correlation with disease activity score in 28 joints (DAS28) and carotid intima-media thickness (CIMT). Methods: A total of 82 patients with RA from June 2016 to April 2020 were selected as RA group. Another 35 healthy volunteers were selected as the control group. Total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL-C), high density lipoprotein (HDL-C), and lipoprotein a (LPA) were detected. The disease activity of RA group was evaluated by DAS28 score, and CIMT was examined by ultrasound. The blood lipid level, DAS28 score and CIMT of RA group were analyzed. Results: TC, TG, LDL-C, LPA of RA group were higher than those of control group, HDL-C was lower, dyslipidemia rate was 68.29% higher than 22.86% of control group, and the difference was statistically significant (P<0.05); LPA in active RA group was higher than that in inactive RA group, HDL-C was lower, the difference was statistically significant (P<0.05); TC, TG, LDL-C, LPA levels in abnormal CIMT group were higher than those in normal CIMT group, HDL-C was lower, the difference was statistically significant (P<0.05). HDL-C was negatively correlated with DAS28 score, LPA was positively correlated with DAS28 score (r=−0.335, 0.382, P<0.05), TC, TG, LDL-C, LPA levels were positively correlated with CIMT, and HDL-C was negatively correlated with CIMT (r=0.421, 0.435, 0.418, 0.429, −0.437, P<0.05). Conclusion: Lipid metabolism disorder is common in RA patients, and blood lipid level is closely related to DAS28 score and CIMT. Attention should be paid to the monitoring and management of blood lipids in RA patients.
