2型糖尿病合并代谢综合征患者血清GLP-1水平变化及其临床意义

摘要

目的:探索2型糖尿病合并代谢综合征患者血清胰高血糖素样肽1(glucagon-like peptide 1,GLP-1)水平变化和临床意义。方法:纳入2018年11月至2020年11月黄山首康医院收治的2型糖尿病患者24例和2型糖尿病合并代谢综合征患者40例作为观察组,取同时间段体检的21例正常人作为对照组。收集患者性别、年龄、腰围、是否腹型肥胖、糖尿病病程、收缩压、舒张压、糖化血红蛋白(glycated hemoglobin,GHb)、空腹血糖(fasting blood-glucose,FBG)、三酰甘油(triglyceride,TG)、低密度脂蛋白(low-density lipoprotein cholesterol,LDL-C)、高密度脂蛋白(high-density lipoprotein cholesterol,HDL-C)、总胆固醇(total cholesterol,TCHO)和肿瘤坏死因子α(tumor necrosis factor α,TNF-α)等信息,比较上述指标在三组患者的差异和GLP-1与TNF-α和其他指标的相关性。结果:三组患者的年龄和性别相比差异无统计学意义(P>0.05)。与对照组相比,糖尿病合并代谢综合征组患者的腰围、BMI、收缩压、舒张压、空腹血糖GHb、TC、TG、HDL-C、TNF-α和GLP-1的水平差异有统计学意义(P<0.05)。与糖尿病组患者比较,糖尿病合并代谢综合征患者的腰围、BMI、收缩压、舒张压、GHb、TG、HDL-C、TNF-α和GLP-1的水平差异有统计学意义(P<0.05)。与GLP-1表达呈负相关的指标为TC、腰围、TNF-α、TG、收缩压、舒张压、GHb和空腹血糖,相关系数r分别为−0.61、−0.54、−0.43、−0.42、−0.35、−0.31、−0.29、−0.24。结论:GLP-1的水平和2型糖尿病进展有关,并且与TNF-α、TC、腰围、TG、收缩压、舒张压、GHb和空腹血糖等指标呈负相关。

Changes and its clinical significance of serum GLP-1 levels in patients with type 2 diabetes mellitus complicated with metabolic syndrome

Abstract

Objective: To explore the changes and clinical significance of serum glucagon-like peptide 1 (GLP-1) level in patients with type 2 diabetes mellitus complicated with metabolic syndrome. Methods: A total of 24 patients with type 2 diabetes and 40 patients with type 2 diabetes combined with metabolic syndrome admitted to our hospital from January 2018 to December 2020 were included in this study. Twenty-one healthy subjects who came to the hospital for physical examination during the same period were selected as the control group. Patients’ gender, age, waist circumference, abdominal obesity, duration of diabetes, systolic and diastolic blood pressure, glycosylated hemoglobin (GHb), fasting blood glucose, triglyceride (TG), low density lipoprotein (LDL-C), high density lipoprotein (HDL-C), total cholesterol and TNF alpha were collected. The differences of the above indexes and the correlation between GLP-1, TNF-α and other indexes in the three groups were analyzed. Results: There was no significant difference in age and sex among the three groups (P>0.05). Compared with the control group, the levels of waist circumference, BMI, systolic blood pressure, diastolic blood pressure, fasting blood glucose glycosylated hemoglobin, TC, TG, HDL-C, TNF-α and GLP-1 in diabetic patients with metabolic syndrome group were different (P<0.05). Compared with diabetic group, waist circumference, BMI, systolic blood pressure, diastolic blood pressure, glycosylated hemoglobin, TG, HDL-C, TNF-α and GLP-1 levels in diabetic patients with metabolic syndrome were significantly different (P<0.05). The expression of GLP-1 was negative correlated with TC, waist circumference, TNF-α, TG, systolic blood pressure, diastolic blood pressure, HbA1c and fasting blood glucose. Conclusion: GLP-1 levels are associated with the progression of type 2 diabetes mellitus, and are negatively correlated with TNF-α, TC, waist circumference, TG, systolic blood pressure, diastolic blood pressure, HbA1c, and fasting blood glucose.
