冠心病患者心电图QRS波时限和血清sST2水平与预后的关系及碎裂QRS波群形成的logistics分析

摘要

目的:分析患者心电图QRS波时限和血清sST2与预后的关系及碎裂QRS波群形成的影响因素。方法:选取苏州大学附属第二医院2019年6月至2020年6月间收治的106例冠心病患者,依据疾病进展程度分为心绞痛组、心肌梗死组、心力衰竭组,50例健康体检者作为对照组。检测受试者血中sST2及心电图QRS波时限;采用logistics回归模型分析fQRS形成的因素。结果:冠心病组患者QRS波时限和sST2水平均明显高于对照组,且差异有统计学意义(P<0.05);心绞痛组、心肌梗死组、心力衰竭组患者QRS波时限和sST2水平差异有统计学意义(P<0.05),其中心力衰竭组患者QRS波时限和sST2水平最高,心绞痛组最低;QRS波时限和sST2水平与冠心病患者疾病进展程度呈正相关(P<0.05);fQRS组患者总胆固醇高于、病变支数多余及Gensini积分明显高于非fQRS组,高密度脂蛋白水平明显低于非QRS组,且差异有统计学意义(P<0.05);冠心病患者病变支数、Gensini积分、高密度脂蛋白低表达是影响fQRS形成的独立性威胁因素(P<0.05)。结论:冠心病患者心电图QRS波时限与血清sST2水平与患者病情严重程度呈正相关,且冠状动脉病变支数以及Gensini积分水平是影响患者出现fQRS的独立影响因素。

Relationship between ECG QRS complex duration, serum sST2 and prognosis in patients with coronary heart disease and the logistic analysis of fragmented QRS complex formation

Abstract

Objective: To analyze the relationship between the patient’s ECG QRS complex duration, serum sST2 and prognosis, and the influencing factors of fragmented QRS complex formation. Methods: A total of 106 patients with coronary heart disease admitted to the Second Afliated Hospital of Soochow University from June 2019 to June 2020 were divided into angina pectoris group, myocardial infarction group, and heart failure group according to the disease progression. Fifty healthy subjects served as the control group. The time limit of sST2 and QRS wave in the blood were detected; the logistics regression model was used to analyze the factors forming fQRS. Results: The QRS wave duration and sST2 of the coronary heart disease group were significantly higher than those of the control group, and the difference was statistically significant (P<0.05). There were significant differences in QRS wave duration and sST2 levels in the angina pectoris group, myocardial infarction group, and heart failure group. And the difference was statistically significant (P<0.05). The QRS duration and sST2 level of patients in the central failure group were the highest, and the angina group was the lowest; QRS duration and sST2 levels were positively correlated with the disease progression (P<0.05); the total cholesterol, the number of lesions and Gensini score in the fQRS group were significantly higher than those in the non-fQRS group, and the high-density lipoprotein level was significantly lower than that in the non-QRS group, and the difference was statistically significant (P<0.05); the number of lesions, Gensini score and low high density lipoprotein were independent risk factors for the formation of fQRS (P<0.05). Conclusion: There is a positive correlation between the ECG QRS wave duration and serum sST2 in patients with coronary heart disease and the severity of the patient's condition. The number of coronary artery lesions and the level of Gensini score are independent factors that affect the appearance of fQRS in patients.
