水疗法对轻中度帕金森患者作用效果的荟萃分析

摘要
目的:系统评价水疗法对轻中度帕金森(Parkinson’s disease,PD)患者平衡能力、功能迁移能力、运动能力、活动耐力、步态、疼痛和生存质量的作用效果。方法:计算机检索Web of Science、PubMed、Embase、The Cochrane Library、Ovid、CNKI、WanFang Data和VIP Database for Chinese Technical Periodicals中有关水疗法对轻中度帕金森患者平衡能力、功能迁移能力、运动能力、活动耐力、步态、疼痛和生存质量影响的随机对照试验(randomized controlled trial,RCT),检索时限为各数据库建库至2019年3月1日。由2名研究人员独立筛选文献,提取资料,并评价纳入研究的偏倚风险,采用RevMan5.3软件进行Meta分析。结果:共纳入11个RCT,共328例患者。与对照组相比,水疗法能提高轻中度帕金森患者的平衡能力(MD=3.12,95%CI:1.34~4.89,P=0.0006),改善患者的功能迁移能力(MD=–1.36,95%CI:–1.96~–0.76,P<0.00001),降低患者的疼痛(MD=–1.20,95%CI:–1.81~–0.59,P=0.0001),提高患者的生存质量(MD=–6.35,95%CI:–12.17~–0.54,P=0.03)。但在改善患者运动能力、活动耐力和步态上,两组差异无统计学意义(P>0.05)。结论:当前证据显示,与陆地训练或常规护理相比,水疗法干预对改善轻中度帕金森患者的平衡能力、功能迁移能力、疼痛和生存质量方面均有积极作用。受纳入研究数量和质量限制,上述结论尚需开展更多高质量研究予以验证。

Efficacy of hydrotherapy in mild-to-moderate Parkinson’s disease: A Meta-analysis

Abstract
Objective: To systematically review the efficacy of hydrotherapy in patients with mild-to-moderate Parkinson’s disease (PD). Methods: To obtain data of randomized controlled trials (RCTs) which studied the efficacy of hydrotherapy for PD, we searched Web of Science, PubMed, EMbase, The Cochrane Library, Ovid, CNKI databases, WanFang Data and VIP Database for Chinese Technical Periodicals from databases inception to March 1st, 2019. Literature screening, data extraction, and the risk of bias assessment for all eligible studies were conducted by two researchers independently. Meta-analysis was performed using RevMan 5.3 software. Results: Our search identified 233 articles, of which 11 RCTs with a total of 328 patients were included for analysis. Results showed that hydrotherapy could significantly improve balance ability (MD=3.12, 95% CI: 1.34 to 4.89, P=0.0006), functional mobility (MD=–1.36, 95% CI: –1.96 to –0.76, P<0.00001), release pain (MD=–1.20, 95% CI: –1.81 to –0.59, P=0.0001), and quality of life (MD=–6.35, 95% CI: –12.17 to –0.54, P=0.03) of mild-to-moderate PD patients. However, there was no statistically significant difference between two groups on motor function, exercise tolerance and freezing of gait (all P values >0.05). Conclusion: Hydrotherapy is efficacious in improving mild-to-moderate PD patients’ balance ability, functional mobility, quality of life, and releasing pain. Because of the limitation of quantity and quality of included studies, more high-quality studies are needed to verify the above conclusion.
