侵袭性肺曲霉菌病检测方法进展

摘要

烟曲霉是导致侵袭性肺曲霉菌病(invasive pulmonary aspergillosis,IPA)的主要致病菌。免疫功能低下的患者更容易发生这种病变,且病死率很高。早期、准确的诊断和治疗对于患者的预后至关重要。目前用于检测这种真菌感染的诊断方法是常规真菌学检查(直接显微镜检查,组织学检查和培养)、影像学、非培养基检测半乳甘露聚糖和(1,3)-β-D-葡聚糖和细胞外糖蛋白以及基于PCR的分子检测等。上述方法的灵敏度和特异性较低,有待开发新的诊断工具和方法,以提高高危患者侵袭性肺曲霉病的快速诊断。

Abstract

Aspergillus fumigatus is the major causative agent of invasive pulmonary aspergillosis (IPA). Patients with low immune function are more likely to develop this disease with a high mortality rate. Early, accurate diagnosis and treatment are critical to the patient's prognosis. The current diagnostic methods for detecting this fungal infection are conventional mycological examination (direct microscopy, histological examination and culture), imaging, non-media detection of galactomannan, β(1,3)-glucan sugar and extracellular glycoproteins, as well as PCR-based molecular detection, but the sensitivity and specificity of appeal methods are low, so new diagnostic tools and methods have been developed to improve the rapid diagnosis of invasive pulmonary aspergillosis in high-risk patients.
