慢性心力衰竭患者血清CK-MB,cTnT与病情严重程度及临床预后的关系

摘要

目的:探究慢性心力衰竭(chronic heart failure,CHF)患者血清肌酸激酶同工酶(creatine kinase isoenzyme,CK-MB)及肌钙蛋白(troponin,CTn)T水平与病情严重程度及临床预后的相关性。方法:抽取2017年2月至2018年6月成都市第六人民医院心内科收治的240例CHF患者的临床资料,其中纽约心脏病协会(New York Heart Association,NYHA)心功能II级66例、III级98例、IV级76例。入院后测定CK-MB及cTnT含量。患者均予标准化治疗,出院后保持随访以统计预后情况。结果:Pearson分析显示血清CK-MB,CTnT含量与心功能分级、左室Tei指数及血清B型尿钠肽(B-type natriuretic peptide,BNP)水平呈正相关,与左室射血分数(left ventricular ejection fraction,LVEF)呈负相关(P<0.05)。240例患者中位随访时间19个月,65例因HF加重再次入院,11例心源性死亡,共76例(31.67%)预后不良。Cox多因素回归分析显示心功能分级、LVEF、左室Tei指数、BNP、CK-MB、CTnT是预后不良的危险因素(P<0.05)。结论:血清CK-MB,cTnT与CHF患者病情严重程度及临床预后密切相关,可作为临床重点监测指标。

Correlation of serum CK-MB and cTnT with severity and clinical prognosis in patients with chronic heart failure

Abstract

Objective: To explore the relationship between serum creatine kinase isoenzyme (CK-MB) and troponin (CTN) T levels and severity of chronic heart failure (CHF) and clinical prognosis. Methods: The clinical data of 240 CHF patients who were admitted to the department from February 2017 to June 2018 were collected. According to cardiac function grading of New York Heart Association (NYHA), there were 66 cases at grade II, 98 cases at grade III, and 76 cases at grade IV. The CK-MB and cTnT levels were measured after admission. All were given standardized treatment. After discharge, follow-up was conducted to statistically analyze prognosis. Results: Pearson analysis showed that serum CK-MB and CTnT levels were positively correlated with cardiac function grading, left ventricular Tei index and B-type natriuretic peptide (BNP), while negatively correlated with left ventricular ejection fraction (LVEF) (P<0.05). The median follow-up time of 240 patients was 19 months. There were 65 cases of re-admitted due to HF aggravation, and 11 cases with cardiac death. There were 76 case (31.67%) with poor prognosis. Cox multivariate regression analysis showed that cardiac function grading, LVEF, left ventricular Tei index, BNP, CK-MB and CTnT were risk factors for poor prognosis (P<0.05). Conclusion: Serum CK-MB and cTnT are closely related to the severity and clinical prognosis of CHF patients, which can be used as key clinical monitoring indicators.
