以家庭为中心的护理干预联合生物反馈治疗对儿童功能性便秘的疗效影响

摘要
目的:探讨以家庭为中心的护理联合生物反馈治疗对儿童功能性便秘(functional constipation,FC)的疗效以及生活质量的影响。方法:选取湖南省儿童医院门诊诊断为FC的4~12岁患儿共58例,随机分为对照组29例,观察组29例。两组均采用常规护理和生物反馈治疗,对观察组加以家庭为中心的护理,采用生活质量量表和临床症状积分表分别对两组患儿于干预前、干预后、随访时进行评分。结果:与干预前相比,干预后两组患儿在社交功能、排便临床症状均有明显的好转,观察组在情感方面相对于对照组改善更明显(P<0.05)。随访时,观察组在粪便性状与排便伴随症状保持得较好,得分低于对照组(P<0.05)。干预后,观察组有效率为96.5%,对照组有效率为93.1%,两组无统计学意义(P>0.05)。随访2个月后,观察组有效率为93.1%,对照组有效率为82.7%,两组有效率均有下降,存在统计学差异(P<0.05)。结论:以家庭为中心的护理联合生物反馈治疗可提高儿童FC的治愈率,同时可提高患儿和家庭的生存质量。

Abstract
Objective: To explore the effect of family-centered nursing combined with biofeedback therapy on children’s functional constipation and quality of life. Methods: A total of 58 children aged 4–12 years old from an outpatient clinic of Hunan Provincial Children’s Hospital were divided into observation group (29 cases) and control group (29 cases) according to Rome III diagnosis of functional constipation. The control group received routine nursing and biofeedback treatment, while the observation group received family-centered nursing on the basis of routine nursing. The quality of life scale and clinical symptom score were used to score the two groups before, after and during the follow-up. Results: Compared with before intervention, after intervention, the social function and clinical symptoms of defecation of the two groups were significantly improved, and the emotional improvement of the observation group was more obvious than that of the control group (P<0.05). During the follow-up, the observation group maintained better fecal characteristics and accompanying symptoms of defecation, and the score was lower than that of the control group (P<0.05). After intervention, the observation group was 96.5% and the control group was 93.1%, no statistical differences between the two groups (P>0.05). During follow-up, the observation group was 93.1% and the control group was 82.7%, there are statistical differences between the two groups (P<0.05). Conclusion: Family-centered nursing combined with biofeedback therapy can improve the cure rate of children with functional constipation, and improve the quality of life of children and families.
