卵巢子宫内膜样腺癌组织中细胞角蛋白、CA125、抑制素α的表达及临床意义

摘要

目的:观察卵巢子宫内膜样腺癌组织中细胞角蛋白(CKpan)、糖类抗原125(CA125)、抑制素α(inhibin α)表达水平,探究其临床意义。方法:选取2012年1月至2015年1月河南科技大学第一附属医院收治的卵巢子宫内膜样腺癌患者73例,通过手术取其卵巢子宫内膜样腺癌组织。另选择
73例因宫颈癌切除卵巢且无卵巢转移患者的正常卵巢组织作为对照。采用免疫组织化学染色法检测CKpan,CA125和inhibin α的表达水平,分析CKpan,CA125,inhibin α与卵巢子宫内膜样腺癌临床病理特征及预后关系。结果:正常卵巢组织、子宫内膜异位症卵巢组织、卵巢子宫内膜样腺癌组织中CKpan和CA125表达率依次升高,两两比较差异有统计学意义(P<0.05);正常卵巢组织、子宫内膜异位症卵巢组织、卵巢子宫内膜样腺癌组织中inhibin α表达率依次降低,两两比较差异有统计学意义(P<0.05);CKpan与CA125蛋白表达呈正相关(r=0.476,P<0.05),CKpan与inhibin α蛋白表达呈负相关(r=−0.493,P<0.05),CA125与inhibin α蛋白表达呈负相关(r=−0.473,P<0.05)。CKpan,CA125,inhibin α蛋白表达均与分化程度、肌层浸润、FIGO分期、淋巴结是否转移有关(P<0.05);CKpan,CA125高表达组3年存活率、中位生存时间均显著低于低表达组(P<0.05);inhibin α低表达组3年存活率、中位生存时间显著低于高表达组(P<0.05)。低分化、FIGO分期III~IV、淋巴结转移、CKpan高表达、CA125高表达是影响卵巢子宫内膜样腺癌患者预后的独立危险因素(均P<0.05);inhibin α高表达是独立保护因素(P<0.05)。结论:CKpan,CA125在卵巢子宫内膜样腺癌组织中高表达率高,inhibin α高表达率低,三者与卵巢子宫内膜样腺癌患者分化程度、肌层浸润、FIGO分期、淋巴结转移等临床病理参数及预后密切相关,可能参与卵巢子宫内膜样腺癌发生发展。

Expressions and clinical significances of CKpan, CA125 and inhibin α in ovarian endometrioid adenocarcinoma

Abstract

Objective: To observe the expressions of cytokeratin (CKpan), carbohydrate antigen 125 (CA125) and inhibin α in ovarian endometrioid adenocarcinoma and explore their clinical significances. Methods: Seventy-three patients with ovarian endometrioid adenocarcinoma admitted to our hospital from January 2012 to January 2015 were selected and their ovarian endometrioid adenocarcinoma tissues were obtained by operation. Another 73 normal ovarian tissues from patients with cervical cancer who had ovariectomized without ovarian metastasis were selected as controls. The expression levels of CKpan, CA125 and inhibin α were detected by immunohistochemical staining, and the relationships between CKpan, CA125, inhibin α and clinicopathological features, prognosis of ovarian endometrioid adenocarcinoma were analyzed. Results: The positive rates of CKpan and CA125 in ovarian endometrioid adenocarcinoma tissues were significantly higher than those in normal ovarian tissues (P<0.05), and the positive rates of inhibin α in ovarian endometrioid adenocarcinoma tissues were significantly lower than those in normal ovarian tissues (P<0.05). The expressions of CKpan, CA125 and inhibin α were correlated with the degree of differentiation, myometrial invasion, FIGO stage and lymph node metastasis (P<0.05). The 3-year survival rate and median survival time of CKpan and CA125 positive expression groups were significantly lower than those of negative expression groups (P<0.05), while the 3-year survival rate and median survival time of inhibin α negative expression group were significantly lower than those of positive expression group (P<0.05). Poor differentiation, FIGO stage III-IV, lymph node metastasis, CKpan positive expression and CA125 positive expression were independent risk factors affecting the prognosis of patients with ovarian endometrioid adenocarcinoma (all P<0.05), and inhibin α positive expression was an independent protective factor (P<0.05). Conclusion: CKpan and CA125 are highly expressed in ovarian endometrioid adenocarcinoma, while inhibin α is low, they are closely related to clinicopathological parameters and prognosis of ovarian endometrioid adenocarcinoma, such as differentiation degree, myometrial invasion, FIGO stage, lymph node metastasis, and may be involved in the occurrence and development of ovarian endometrioid adenocarcinoma.
