摘要

目的:研究2型糖尿病(type 2 diabetes mellitus,T2DM)患者中性粒细胞/淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)与踝肱指数(ankle brachial index,ABI)的关系,分析NLR与下肢血管病变之间的相关性。方法:共纳入683例T2DM患者,根据ABI水平分为外周血管病变(peripheral arterial diseases,PAD)组(ABI<0.9)和无外周血管病变组(ABI≥0.9),比较两组间NLR及临床资料的差异,分析T2DM患者发生PAD的危险因素。结果:ABI异常组年龄、病程、收缩压、糖化血红蛋白(HbA1c)、中性粒细胞、NLR、血肌酐(creatinine,Cr)、冠心病(coronary heart disease,CHD)和脑梗死患病率均高于ABI正常组(B组)。二分类Logistic回归分析结果示:高龄、收缩压升高、HbA1c升高、NLR升高、CHD均是ABI异常的独立危险因素,NLR与PAD之间有相关性。结论:NLR是T2DM患者下肢动脉病变的危险因素,可作为糖尿病下肢血管病变的预测指标。

Abstract

Objective: To investigate the association between neutrophil-to-lymphocyte ratio (NLR) with ankle brachial index (ABI) in patients with type 2 diabetes mellitus (T2DM), and to analyze the correlation between NLR and peripheral arterial disease. Methods: A total of 683 patients with T2DM were enrolled in this study and divided into two groups according to ABI value of peripheral arterial diseases (PAD) group (ABI<0.9) and non-PAD group (ABI≥0.9). NLR and clinical data were compared between the two groups. The risk factors for incidence of PAD in patients with T2DM were analyzed. Results: Age, disease course, systemic blood pressure, HbA1c, neutrophile, NLR, serum creatinine, coronary heart disease and cerebral infarction prevalence in ABI abnormal group were higher than those in ABI normal group. Binary Logistic regression analysis showed that advanced age, elevated systolic blood pressure, elevated HbA1c, elevated NLR, and coronary heart disease were independent risk factors for ABI abnormality, and NLR value was significantly correlated with PAD. Conclusion: NLR is a risk factor for lower limb arterial disease in patients with T2DM and can predict the occurrence of diabetic lower extremity vascular disease.
