高危型人乳头状瘤病毒多重感染在宫颈鳞状细胞病变中的分布及意义

摘要
目的:探讨高危型人乳头状瘤病毒(high-risk human papilloma virus,HR-HPV)多重感染在宫颈鳞状细胞病变中的分布特点及临床意义,为宫颈病变的诊断、治疗以及预防性接种疫苗提供新的理论依据。方法:收集2011至2015年39 331例女性患者的宫颈HPV分型检测、液基细胞学(liquid based cytology,LCT)检查及部分宫颈活检诊断结果,分析HR-HPV多重感染与宫颈鳞状细胞病变患者的临床病理特征的关系。结果:在39 331例患者中,HR-HPV多重感染者768例,年龄为(38.10±10.28)岁,2重感染及3重感染主要集中在21~50岁,4重以上感染主要集中在21~40岁。应用卡方检验进一步分析可以发现,2重感染、3重感染和4重以上感染在各年龄组的分布存在差异(χ2=23.80,P=0.008);卡方检验风险分析显示,人群中HR-HPV多重感染增加宫颈鳞状细胞学异常发病的风险(OR=1.32,P=0.01);Spearman等级相关分析显示HR-HPV多重感染与宫颈鳞状细胞病变级别呈正相关(P=0.01)。结论:HR-HPV多重感染增加了宫颈癌前病变的发生及癌变风险,在诊断、治疗和预防宫颈病变时,应重点参考HR-HPV的多重感染情况。

Abstract
Objective: To probe e distribution of high-risk human papilloma virus (HR-HPV) multiple infection in cervix squamous cell and to provide new theoretical basis and clinical reference for diagnosis, treatment and preventive vaccination. Method: A retrospective study was performed, which involved 39 331 cases during 2011 to 2015 from Shantou Central Hospital using cervical HPV testing, liquid based cytology (LCT) and part of the biopsy, focus on analysis the relationship between multiple infections and clinicopathological features of patients with cervical squamous cell disease Results: HR-HPV multiple infection was 768 cases, and the average age was 38.10±10.28 years old. The age was focus on 21–50 years old in double or triple infection, 21–40 years old in quad infection. There was difference in the distribution of each age group of Chi-square test (χ2=23.80, P=0.008). The multiple high risk-HPV infection increases the risk of abnormal of LCT scene (OR=1.32). HR-HPV multiple infection was positively correlated with cervical squamous cell lesion grade of Spearman rank correlation analysis. Conclusion: HR-HPV multiple infections increase the incidence of cervical precancerous lesions and the risk of cancer. we should focus on the multiple infection of HR-HPV in the diagnosis, treatment and prevention of cervical lesions.
