宫颈液基细胞学联合高危型HPV E6/E7 mRNA检测在宫颈病变筛查中的应用

摘要
目的:探讨宫颈液基细胞学(thinprep cytologic test,TCT)联合高危型HPV E6/E7 mRNA检测在宫颈病变筛查中的应用价值。方法:选择2016年8月至2018年10月福建医科大学附属闽东医院及分院妇科同时行TCT及高危型HPV E6/E7 mRNA检测的12 679例患者,结合组织病理学活检诊断为上皮内病变或癌的病例进行数据分析。结果:12 679例患者中,HPV E6/E7 mRNA阳性者1 808例,阳性率14.26%,TCT异常者1 054例。在组织病理学活检异常病例中,HPV检出率92.7%,TCT及HPV两者任意阳性占98.2%,16型HPV阳性占高危型HPV阳性18.18%。在腺上皮异常病例中,18/45型HPV阳性占48.28%。联合检测的敏感性98.2%,特异性91.5%。结论:高危型HPV E6/E7 mRNA检测在宫颈上皮内病变及癌中具有较高的敏感性,联合TCT检测可提高宫颈病变的检出率。

Application of cervical thinprep cytologic test combined with high-risk HPV E6/E7 mRNA detection in the screening of cervical lesions

Abstract
Objective: To study the clinical value of thinprep cytologic test (TCT) combined with high-risk HPV E6/E7 mRNA detection in the screening of cervical lesions. Methods: From August 2016 to October 2018, a total of 12 679 cases who had received TCT and high-risk HPV E6/E7 mRNA test at Mindong Hospital Affiliated to Fujian Medical University and the branch hospital were selected. Statistical analysis of cases which were diagnosed as cervical intraepithelial lesions or cancer by histopathological biopsy. Results: Among 12 679 patients, 1 808 cases were found HPV E6/E7 mRNA positive. The positive rate was 14.26%. A total of 1 054 cases got abnormal TCT results. In cases diagnosed with cervical intraepithelial lesions or cancer by histological pathology, the detection rate of HPV was 92.7%; 98.2% were positive for either TCT or HPV; HPV16 accounted for 18.18% of high-risk HPV. In abnormal cases of glandular epithelium, 48.28% were positive for 18/45 HPV. Sensitivity and specificity of combined testing were 98.2% and 91.5%. Conclusion: The sensitivity of high-risk HPV E6/E7 mRNA test was higher in cervical lesions and cancer. Combined high-risk HPV E6/E7 mRNA test with TCT test could enhance the detection rate of cervical lesions.
