风湿性二尖瓣狭窄患者心肌LKB1/AMPK 通路与心房颤动的相关性

摘要
目的:探讨风湿性心脏病二尖瓣狭窄患者肝激酶B1/丝裂原活化蛋白激酶(liver kinase B1/AMPactivated protein kinase,LKB1/AMPK)通路与伴发心房颤动(atr ial f ibri l lation,AF)的关系。 方法:选择2011年12月至2016年12月行换瓣手术治疗的风心病二尖瓣狭窄患者40例,根据病史 和术前心电图记录分为房颤组(AF组,n=20)和窦性心律(sinus rhythm,SR)组(SR组,n=20), 术前抽取空腹静脉血检测生化指标,术中留取左心耳组织。ELISA法测定血清超敏C反应蛋白 (hypersensitive C-reactive protein,hs-CRP)及硫氧还蛋白(thioredoxin,TRX)的水平。实时定量 PCR(quantitative real-time PCR,RT-PCR)法检测左心耳组织中LKB1和AMPK的mRNA水平。采 用Pearson相关分析检测LKB1与各指标的相关性。结果:与SR组相比,AF组患者左心耳组织中 LKB1及AMPK的mRNA水平明显减低(均P<0.05);血清中hs -CRP及TRX水平显著高于SR组(均 P<0.05)。Pearson相关分析提示LKB1与AMPK呈正相关(r=0.373,P<0.05),与hs-CRP,TRX呈负 相关(r=−0.546,−0.417,均P<0.05) 结论:LKB1/AMPK通路可通过炎症及氧化应激机制触发和 维持AF。

Correlation between LKB1/AMPK pathway and atrial fibrillation in patients with mitral stenosis of rheumatic heart disease

Abstract
Objective: To investigate the correlation between the liver kinaseB1/AMP-activated protein kinase pathway (LKB1/AMPK pathway) and atrial fibrillation (AF) in patients with mitral stenosis of rheumatic heart disease (RHD). Methods: A total of 40 patients with RHD who underwent valve replacement surgery were enrolled, including 20 with persistent AF and 20 with sinus rhythm (SR). Peripheral venous blood samples were taken before surgery and cardiac muscle tissue were taken from the right atrial appendages during surgery. ELISA was used to exam the level of hypersensitive C-reactive protein (hs-CRP) and thioredoxin (RTX) in peripheral venous blood. Real-time reverse transcription polymerase chain reaction (RT-PCR) was used to analyze the expressions of LKB1 and AMPK in the 40 patients. The correlation between AMPK, hs-CRP, RTX and LKB1 were analysed. Results: Compared with SR group, mRNA levels of LKB1 and AMPK were elevated in cardiac tissues from AF patients (P<0.05). The level of hs-CRP and RTX were also elevated in AF patients. Pearson correlation analysis showed that LKB1 was positively correlated with AMPK (r=0.373, P<0.05), and negatively correlation with hs- CRP, TRX (r=−0.546, −0.417, all P<0.05). Conclusion: LKB1/MAPK pathway may play an important role in the initiation and progression of AF through inflammation and oxidative stress.
