运动干预对造血干细胞移植患者生命质量影响的Meta分析

摘要
目的:通过Meta分析探讨运动干预对造血干细胞移植(hematopoietic stem cell transplantation,HSCT)患者生命质量的影响。方法:计算机检索Cochrane Library,PubMed,Web of Science,Elsevier,中国生物医学文献数据库,中国知网及万方数据库中使用运动干预改善HSCT患者生命质量的随机对照研究,由2名研究者分别评价文献质量后,采用Review Manager 5.3进行统计分析。结果:共纳入8篇文章,采用欧洲癌症研究与治疗组织开发的生命质量测定量表(European Organization for Research and Treatment of Cancer Quality of Life Questionnaire,EORTC-QLQ-C30)作为生命质量测评工具进行Meta分析,结果显示:运动干预可显著提高HSCT患者总体健康状况(WMD=5.63,95% CI:2.42~8.83,P<0.001),并对其功能(WMD=4.55,95% CI:2.78~6.32,P<0.001)及症状领域(WMD=−4.55,95% CI:−6.26~−2.84,P<0.01)具有明显改善作用。结论:运动干预对提高HSCT患者生命质量有积极的影响作用,为今后开展针对HSCT患者运动干预实践提供了理论依据。

Abstract
Objective: To explore the effects of physical exercise for quality of life of patients undergoing hematopoietic stem cell transplantation (HSCT) through Meta-analysis. Methods: Randomized controlled trials (RCTs) of physical exercise on quality of life were searched in databases including Cochrane Library, PubMed, Web of Science, Elsevier, CBM, CNKI, Wanfang Data. After two researchers evaluated the literature quality, Review Manager 5.3 was used to perform the Mata-analysis. Results: Eight RCTs were retrieved and considered as eligible. According to the meta-analysis which concluded by RCTs using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC-QLQ-C30), physical exercise could significantly improve global quality of life (WMD=5.63, 95% CI: 2.42 to 8.83), P<0.001), physical functioning, emotional functioning, cognitive functioning (WMD=4.55 95% CI: 2.78 to 6.32), P<0.001) and general symptoms (WMD=−4.55,
95% CI: −6.26 to −2.84, P<0.01). Conclusion: Physical exercise could effectively improve quality of life in HSCT survivors and provide a theoretical basis for the practice of HSCT patients’ exercise intervention in the future.
