家庭无创正压通气在慢性左室心力衰竭中的疗效

摘要
目的:探讨家庭无创正压通气在慢性左室心力衰竭中的治疗作用。方法:选择2016年1月至12月 于保定市第一医院就诊的51例慢性左室心力衰竭患者进行观察,根据患者是否同意使用呼吸机 分为两组:对照组给予常规抗心力衰竭治疗,治疗组给予常规抗心力衰竭和家庭无创正压通气 治疗,治疗1年后观察两组临床症状、动脉血氧分压(arterial partial pressure of oxygen,PaO2)、 6分钟步行试验(6-minute walking test,6MWT)、脑钠肽(brain natriuretic peptide,BNP)水平、 左室射血分数(left ventricular ejection fraction,LVEF)。结果:治疗1年后,治疗组临床症状、 PO2、6MWT、BNP水平、LVEF均有显著改善,明显优于对照组,差异有统计学意义(P<0.05)。 结论:家庭无创正压通气有助于改善慢性左室心力衰竭患者的心功能,维持心功能的稳定,提 高患者生活质量。

Effect of home non-invasive positive pressure ventilation in patients with chronic left heart failure

Abstract
Objective: To explore the therapeutic effect of home noninvasive positive pressure ventilation in patients with chronic left heart failure. Methods: We selected 51 patients with chronic left heart failure who were admitted to our hospital from January 2016 to December 2016. Patients were divided into a control group and a treatment group according to whether they agreed to use the ventilator. The control group was given conventional anti heart failure treatment. The treatment group was treated with conventional anti heart failure and non-invasive positive pressure ventilation. After 1-year treatment, the clinical symptoms, arterial partial pressure of oxygen (PaO2), the 6-minute walking test (6MWT), the level of brain natriuretic peptide (BNP) and left ventricular ejection fraction (LVEF) were observed. Results: After 1-year of treatment, the clinical symptoms, PaO2, 6MWT, BNP and LVEF improved significantly in the treatment group, which were significantly better than that in the control group (P<0.05). Conclusion: Home noninvasive positive pressure ventilation can improve the heart function of patients with chronic left heart failure, maintain the stability of heart function and improve the quality of life of patients.
