MMP-2、MMP-9与溃疡性结肠炎患者肠黏膜通透性的相关性研究

摘要

目的:观察血浆基质金属蛋白酶MMP-2、MMP-9表达水平与溃疡性结肠炎(ulcerative colitis,UC)患者肠黏膜通透性的联系,探讨UC的发病机制,为诊断提供较为可靠的参考指标。方法:纳入
50例UC患者和30例健康志愿者,分别采集研究对象的外周静脉血,应用酶联免疫吸附法(ELISA)检测血浆MMP-2、MMP-9的表达,高效液相色谱法检测口服乳果糖(L)和甘露醇(M)混合液6 h内尿液L、M的含量,并计算乳果糖/甘露醇比值(LMR)。结果:与健康对照组相比,UC组血浆MMP-2、MMP-9水平显著升高 (t=13.843,P<0.001;t=14.165,P<0.001)。UC组血浆MMP-2、MMP-9表达与疾病病变严重程度、病变范围呈正相关;UC组患者尿LMR显著高于健康对照组(t=9.804,P<0.001),UC患者血浆MMP-2、MMP-9表达与肠黏膜通透性呈一定程度的相关性(r=0.832,P<0.001;r=0.847,P<0.001)。结论:UC患者血浆MMP-2、MMP-9表达升高,并能反映UC病变严重程度、病变范围,作用机制可能与肠黏膜通透性增高有关。

Abstract

Objective: To investigate the relationship between the expression of matrix metalloproteinases (MMPs) and intestinal mucosal permeability in patients with ulcerative colitis. Methods: A total of 50 UC patients and 30 healthy volunteers were enrolled in this study. The levels of MMP-2 and MMP-9 in the serum were assessed by ELISA. Detected the lactulose and mannitol in participants after they took orally the lactulose and mannitol test solution by HPLC and calculated the ratio of lactulose/mannitol. Results: The serum levels of MMP-2 and MMP-9 in UC patients were significantly higher than that in healthy controls (t=13.843, P<0.001; t=14.165, P<0.001). The levels of MMP-2 and MMP-9 were correlated with the severity and extent of disease (P<0.001). The lactulose/mannitol ratio in UC patients was significantly higher than in control group (t=9.803, P<0.001).The serum levels of MMP-2 and MMP-9 in UC patients were correlated with the change of intestinal mucosal permeability. The serum levels of MMP-2 and MMP-9 in UC group are significantly increased, which is correlated with the change of intestinal mucosal permeability(r=0.832, P<0.001; r=0.847, P<0.001). Conclusion: The serum levels of MMP-2 and MMP-9 in UC patients are significantly increased, which is correlated with the change of intestinal mucosal permeability.
