组织细胞样乳腺癌临床病理学特征研究及文献复习

摘要

目的:观察组织细胞样乳腺癌的临床病理、免疫组织化学特点及鉴别诊断,并复习相关文献。 方法:收集北大人民医院病理科2007~2014年及航空总医院病理科,建科以来的组织细胞样乳腺癌3 例,分析其临床病理学及免疫组织化学特征,并采用FISH检测HER2基因的扩增水平。结果:镜下肿瘤细胞呈小巢或条索状生长。肿瘤细胞呈多边形,圆形,胞浆丰富,嗜酸,呈颗粒状、泡沫状,组织细胞样,细胞核呈圆形及卵圆形,染色质细腻,核仁不清楚,核轻度异型性,核分裂像不明显。免疫组织化学结果显示:所有3例样本CK(+)、CK7(+)、S100(–)、E-cadherin(–)、GCDFP 15(+)、CD68(–)。其中1例进行了HER2 FISH检测,结果为阴性。结论:组织细胞样乳腺癌是一种少见的特殊类型乳腺癌,在日常的诊断工作中,需结合免疫组织化学检查,以便排除其它肿瘤的可能性和明确诊断。

The investigation of histiocytoid breast carcinoma pathologic features and related literature review

Abstract

Objective: To explore the clinicopathologic features, immunophenotype and differential diagnosis in patients with histiocytoid breast carcinoma and also review the related literature. Method: Three cases diagnosed as histiocytoid breast carcinoma with available slides and clinical information, coded between 2007 and 2014, and was retrieved from Department of Pathology in Aviation General Hospital and Peking University People’s Hospital. The clinical pathologic and immunohistochemistry features were analyzed. The FISH technique was used to detect the level of HER2 gene amplification. Results: Microscopically, the tumor tissue showed cord and small nested-like growth pattern. The cytoplasm was abundant, amphophilic, granular. The nuclei were round-oval and hyperchromatic with inconspicuous nucleoli and finely dispersed chromatin. There was minimal nuclear pleomorphism, and mitotic figures were inconspicuous. The immunohistochemistry results were as follows in three cases: CK(+), CK7(+), S100(−), E-cadherin(−), GCDFP 15(+), CD68(–). FISH detection revealed HER2 was negative in one case. Conclusion: Histiocytoid carcinoma of the breast is a rare tumor and we need combine immunohistochemistry to make a clear diagnosis.
