血清sST2及galectin-3对心房颤动射频消融术后复发的预测价值

摘要

目的:探究血清可溶性致癌抑制因子2(soluble suppression tumorigenicity-2,sST2)、半乳糖凝集素-3(galectin-3,Gal-3)对心房颤动(atrial fibrillation,AF)射频消融术(radiofrequency ablation,RFA)后复发的预测价值。方法:选取皖北煤电集团总医院2019年1月至2021年2月收治的107例AF患者为研究对象,根据RFA后AF复发结果将其分为复发组(n=33)与未复发组(n=74)。收集患者的一般资料及血清sST2、Gal-3等生化指标水平,用单因素及logistic回归模型分析各项指标与AF复发的关系,用受试者操作特征 (receiver operating characteristic,ROC)曲线分析血清sST2、Gal-3对RFA后AF复发的预测价值。结果:复发组的左心房内径(left atrial diameter,LAD)及血清sST2、Gal-3水平均明显高于未复发组(均P<0.05)。Logistic回归分析显示:LAD(OR=1.373)、sST2(OR=1.331)、Gal-3(OR=2.349)均是RFA后AF复发的独立影响因素(均P<0.05)。ROC曲线分析显示:血清sST2、Gal-3水平及LAD预测RFA后AF复发的曲线下面积(area under the curve,AUC)分别为0.831、0.840及0.797;且以sST2联合Gal-3的预测效能最大[AUC=0.944,敏感度=87.88%,特异度=91.89%]。结论:血清sST2、Gal-3均是RFA后AF复发的独立影响因素,可独立预测术后复发,且以二者联合的预测价值最大。

Predictive value of serum sST2 and galectin-3 on recurrence of atrial fibrillation after radiofrequency ablation

Abstract

Objective: To investigate the predictive value of serum soluble suppression of tumorigenicity-2 (sST2) and galectin-3 (Gal-3) for recurrence of atrial fibrillation (AF) after radiofrequency ablation (RFA).
Methods: A total of 107 patients with AF admitted to General Hospital of Wanbei Coal and Electricity Group from January 2019 to February 2021 were selected as the study subjects. According to the recurrence results of AF after RFA, the patients were divided into a recurrence group (n=33) and a non-recurrence group (n=74). The general data and serum sST2, Gal-3 and other biochemical indexes were collected. The relationship between each index and AF recurrence was analyzed by univariate and logistic regression model. The predictive value of serum sST2 and Gal-3 for AF recurrence after RFA was analyzed by receiver operating characteristic (ROC) curve.
Results: The left atrial diameter (LAD), serum sST2 and Gal-3 levels in the recurrence group were significantly higher than those in the non-recurrence group (all P<0.05). Logistic regression analysis showed that LAD (OR=1.373), sST2 (OR=1.331) and Gal-3 (OR=2.349) were independent influencing factors of AF recurrence after RFA (all P<0.05). ROC curve analysis showed that the area under the curve (AUC) of serum sST2, Gal-3 levels and LAD in predicting AF recurrence after RFA were 0.831, 0.840, and 0.797, respectively. The combination of sST2 and Gal-3 had the highest predictive efficiency [AUC=0.944, sensitivity=87.88%, specificity=91.89%].
Conclusion: Serum sST2 and Gal-3 are independent influencing factors of AF recurrence after RFA, which can independently predict postoperative recurrence, and the combination of the two has the highest predictive value.
