吸入用N-乙酰半胱氨酸联合布地奈德治疗小儿肺炎的疗效及其对炎症和免疫功能的影响

摘要
目的:探究吸入用N-乙酰半胱氨酸(N-acetylcysteine,NAC)联合布地奈德治疗小儿肺炎的疗效及其对炎症和免疫功能的影响。方法:选取2019年7月至2022年4月安徽医科大学附属宿州医院收治的108例肺炎患儿为研究对象,按照随机数表法分为对照组(n=54)与观察组(n=54)。对照组吸入治疗用布地奈德,观察组吸入治疗用NAC+布地奈德。比较2组临床疗效、症状消失时间、治疗前后炎症反应[超敏C反应蛋白(high sensitivity C-reactive protein,hs-CRP)、降钙素原(procalcitonin,PCT)、中性粒细胞与淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)]以及免疫功能[免疫球蛋白(immunoglobulin,Ig)A、IgG、IgM]指标水平。结果:与对照组相比,观察组临床总有效率显著提高(92.59% vs 77.78%,P<0.05);观察组咳嗽、啰音、喘息症状消失时间均显著变短(均P<0.05);观察组治疗后血清hs-CRP、PCT、NLR及IgA、IgG、IgM水平均显著降低(均P0.05)。结论:NAC联合布地奈德雾化吸入治疗对小儿肺炎有显著疗效,可改善患儿临床症状,减轻炎症反应,调节免疫功能。

Abstract
Objective: To investigate the effect of inhaled N-acetylcysteine (NAC) combined with budesonide in the treatment of pediatric infantile and its influence on inflammation and immune function.
Methods: A total of 108 children with pneumonia admitted to Suzhou Hospital Affiliated to Anhui Medical University from July 2019 to April 2022 were selected as the research objects and randomly divided into a control group (n=54) and an observation group (n=54). The control group was treated with budesonide inhalation, and the observation group was treated with NAC combined with budesonide inhalation. The clinical efficacy, symptom disappearance time, inflammatory response indexes [high sensitivity C-reactive protein (hs-CRP), procalcitonin (PCT), neutrophil-to-lymphocyte ratio (NLR)] and immune function [immunoglobulin (Ig) A, IgG, IgM] before and after treatment were compared between the 2 groups.
Results: The total effective rate in the observation group was 92.59%, which was significantly higher than 77.78% of the control group (P<0.05). The disappearance time of cough, rales, and gasping in the observation group was shorter than that in the control group (all P<0.05). After treatment, hs-CRP, PCT, NLR, IgA, IgG, and IgM levels in observation group were lower than those in the control group (all P0.05).
Conclusion: The therapeutic effect of aerosol inhalation of NAC combined with budesonide in the treatment of infantile pneumonia is remarkable, which can improve the clinical symptoms, reduce inflammatory response, and regulate immune function.
