红细胞体积分布宽度、降钙素原、脓毒症相关性器官功能衰竭评价评分联合预测脓毒症休克患者短期预后的价值

摘要
目的:探究红细胞体积分布宽度(red cell volume distribution width,RDW)、降钙素原(procalcitonin,PCT)以及脓毒症相关性器官功能衰竭评价(sepsis-related organ failure assessment,SOFA)评分对脓毒症休克患者短期预后的预测价值。方法:回顾性选取2018年7月至2022年1月在六安市中医院接受治疗的脓毒症休克患者101例,根据患者28 d内是否死亡分为2组,存活组61例,死亡组40例。收集所有患者的临床资料,包括一般临床资料以及RDW、PCT水平等。采用logistic回归分析脓毒症休克患者短期死亡的危险因素,受试者操作特征(receiver operating characteristic,ROC)曲线分析RDW、PCT水平及SOFA评分对其短期预后的预测价值。结果:SOFA评分、血清PCT以及RDW是脓毒症休克患者短期内死亡的独立危险因素(均P<0.05)。ROC分析显示:SOFA评分、血清PCT、RDW均对脓毒症休克患者短期预后具有一定的临床预测价值,3个指标联合预测脓毒症休克患者短期预后的敏感度、特异度分别为98.36%、87.50%,约登指数为0.859。结论:RDW或PCT水平偏高的脓毒症休克患者短期内死亡的可能性较高,2个指标均对患者短期预后具有一定的预测价值,此外脓毒症休克患者短期预后还与SOFA评分有关。

Abstract

Methods: A total of 101 patients with septic shock who were treated in Traditional Chinese Hospital of Lu’an from July 2018 to January 2022 were retrospectively selected and divided into 2 groups according to whether the patients died within 28 days, 61 cases in the survival group and 40 cases in the death group. The general clinical data, RDW and PCT levels of patients were collected. The risk factors of short-term death in patients with septic shock were analyzed by logistic regression, and the predictive value of RDW, PCT levels and SOFA score on their short-term prognosis was explored by receiver operating characteristic (ROC) analysis.
Results: The SOFA score, serum PCT level, and RDW were independent risk factors for short-term death in patients with septic shock (all P<0.05). ROC analysis showed that SOFA score, PCT, and RDW had certain clinical predictive values for the short-term prognosis of patients with septic shock, the sensitivity and specificity of the three indexes in predicting the short-term prognosis of patients with septic shock were 98.36% and 87.50%, and the Yoden index was 0.859.
Conclusion: Patients with septic shock with high RDW or PCT level are more likely to die in the short term, both indicators have a certain predictive value for the short-term prognosis of patients. In addition, the short-term prognosis of patients with septic shock is also related to SOFA score.
