肺部霍奇金淋巴瘤误诊为肺癌病例分析

摘要
目的：探讨肺部霍奇金淋巴瘤的临床及病理特点，减少误诊误治。方法：对所获军事医学科学院附属医院病理科诊断为肺部霍奇金淋巴瘤1例病例的临床及病理资料进行回顾性分析。结果：本例因间断咳血，胸部CT示：右肺上叶多发结节，考虑肺癌伴右肺门及纵隔淋巴结转移，给予放射治疗，患者因临床症状进一步加重，来我院诊治，行腹股沟淋巴结穿刺活检术，病理诊断示霍奇金淋巴瘤，经ABVD方案化疗6周期，肿块缩小。结论：肺部霍奇金淋巴瘤临床及病理类型不常见，易误诊为肺癌，应加强对肺部霍奇金淋巴瘤的临床及病理学特征的认识并仔细分析病情，以避免或减少误诊误治。

Abstract
Objective: Though investigating the clinical and pathological features of pulmonary Hodgkin’s lymphoma to reduce misdiagnosis and mistreatment. Methods: Clinical and pathological data were retrospectively analyzed in one case of pulmonary Hodgkin’s lymphoma, which was diagnosed by Affiliated Hospital of Military Medical Sciences. Results: In this case, the patient came to hospital because of intermittent hemoptysis, chest CT was taken and showed: there were multiple nodules on the right, upper lobe, considered lung cancer with right hilar and mediastinal lymph node metastasis. Then the patient was treated with radiotherapy. With clinical symptoms aggravated, the patient came to our hospital for treatment, inguinal lymph node biopsy was taken and pathological diagnosis is Hodgkin’s lymphoma. After 6 cycles of ABVD chemotherapy, the tumor shrank. Conclusion: Clinical and pathological type of pulmonary Hodgkin’s lymphoma is rare, which often misdiagnosed as lung cancer. We should strengthen the clinical and pathological features of pulmonary Hodgkin’s lymphoma awareness and careful analysis of the condition, in order to avoid or reduce misdiagnosis and mistreatment.
