空气压力波联合常规措施预防妇科良性肿瘤患者腹腔镜手术后下肢深静脉血栓形成的效果

摘要

目的:探讨空气压力波联合常规措施对妇科良性肿瘤患者腹腔镜手术后下肢深静脉血栓(deep venous thrombosis,DVT)形成的影响。方法:选取2019年1月至2021年12月在青岛市胶州中心医院拟行腹腔镜手术的妇科良性肿瘤患者200例,采用随机原则分为对照组与观察组,各100例。其中对照组术后给予低分子肝素与逐级加压弹力袜干预治疗,观察组在对照组的基础上联合使用空气压力波治疗,比较两组术后一般情况、凝血功能、股静脉血流速度和DVT发生率。结果:两组术后下床活动时间、引流管留置时间差异无统计学意义(P>0.05),观察组住院时间明显短于对照组(P<0.05)。术后1 d,两组凝血酶原时间(prothrombin time,PT)、活化部分凝血酶时间(activated partial thromboplastin time,APTT)、凝血酶时间(thrombin time,TT)、纤维蛋白原(fibrinogen,FIB)、D-二聚体(D-dimer,D-D)差异均无统计学意义(均P>0.05)。与对照组相比,术后3 d观察组PT、APTT、TT明显延长,FIB、D-D水平显著降低(均P<0.05)。术后3、5及7 d,观察组左股静脉血流速度及右股静脉血流速度显著高于对照组(P<0.05)。观察组术后7 d内DVT发生率为5.00%,显著低于对照组的13.00%,差异有统计学意义(P<0.05)。结论:空气压力波联合常规措施能够有效预防妇科良性肿瘤患者腹腔镜手术后下肢DVT形成。

Effect of air pressure wave combined with routine measures on prevention of deep vein thrombosis of lower extremity in patients with benign gynecological tumor after laparoscopic surgery

Abstract

Objective: To investigate the effect of air pressure wave combined with conventional measures on deep vein thrombosis (DVT) in patients with benign gynecologic tumors after laparoscopic surgery. Methods: A total of 200 patients with benign gynecological tumors scheduled for laparoscopic surgery in our hospital from January 2019 to December 2021 were selected and randomly divided into 2 groups, each group with 100 cases. The control group was treated with low molecular weight heparin and graduated compression stockings. The observation group was treated with air pressure wave on the basis of the control group. The general situation, coagulation function, femoral vein blood flow velocity and DVT incidence were compared between the 2 groups. Results: There was no significant difference in postoperative ambulation time and drainage tube indwelling time between the 2 groups (P>0.05), the hospitalization time of the observation group was significantly shorter than that of the control group (P<0.05). At 1 day after operation, there was no significant difference in coagulation function indexes [prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), fibrinogen (FIB), and D-dimer (D-D)] between the 2 groups (all P>0.05). At 3 days after operation, the PT, APTT, and TT in the observation group were significantly longer than those in the control group (all P<0.05), and FIB and D-D levels were significantly lower than those in the control group (P<0.05). At 3, 5, and 7 d after operation, the blood flow velocity of left femoral vein and right femoral vein in the observation group were significantly higher than those in the control group (P<0.05). The incidence of DVT within 7 d after operation in the observation group was 5.00%, which was significantly lower than 13.00% in the control group (P<0.05). Conclusion: Air pressure wave combined with conventional measures can effectively prevent DVT of Lower Extremity in patients with benign gynecological tumor after laparoscopic surgery.
