2014年中段尿培养病原微生物分布及耐药性分析

摘要
目的:了解并分析2014年南京医科大学第一附属医院中段尿培养阳性标本的病原菌分布、构成及耐药状况,为临床合理使用抗菌药物提供实验依据。方法:细菌、真菌鉴定采用VITEK 2 Compact全自动微生物鉴定系统或API鉴定系统,细菌药物敏感性测定采用Kirby-Bauer纸片扩散法,真菌药物敏感性测定采用ATB FUNGUS 3板条,应用WHONET5.6软件进行统计分析。结果:2014年共送检5 172份中段尿标本,分离出1 326株病原微生物,阳性率25.6%。革兰阴性菌、革兰阳性菌、真菌分别占72.0%、19.6%、8.4%。革兰阴性杆菌对碳青霉烯类抗菌药、革兰阳性球菌对万古霉素和利奈唑胺仍高度敏感;真菌对两性霉素B、伏立康唑高度敏感。检出亚胺培南和美洛培南同时耐药的大肠埃希菌3株、肺炎克雷伯5株;检出5株万古霉素耐药屎肠球菌。结论:大肠埃希菌是泌尿系感染主要致病菌,多种病原菌对临床常用抗菌药物耐药性日趋严重。

Analysis of the distribution and resistance of pathogens isolated cultured from midstream urine in 2014

Abstract
Objective: To investigate the distribution of bacteria and analyze its drug resistance change from the midstream urine in the First Affiliated Hospital of Nanjing Medical University in 2014, so as to provide experimental basis for clinical rational use of antibiotics. Methods: Automatic detection machine of VITEK 2 Compact or API system was used for identification of bacteria and fungus. Kirby-Bauer method was used to test bacteria drug sensitivity and ATB FUNGUS 3 was used to test fungal drug sensitivity. Whonet 5.6 software was used for statistical analysis. Results: In total, 5 172 samples of midstream urine were collected during the year of 2014, and 1 326 ones of them were positive for culturing of pathogenic microorganisms (positive rate, 25.64%). The proportions of pathogenic bacteria were 72.0% for gram negative bacilli, 19.6% for gram positive coccus and 8.4% for fungus respectively. Escherichia coli remained to be one of the most commonly found pathogenic bacteria, of which the proportion of Escherichia coli that producing ESBLs was 61.3%. High sensitivities were also observed with respect to carbapenems for gram negative bacilli, vancomycin and linezolid for gram positive coccus, and amphotericin B and voriconazole for fungus. Besides, three strains of and five strains of Klebsiella pneumoniae resistant to imipenem and meropenem; five strains of Enterococcus faecium resistant to vancomycin were also detected. Conclusion: Escherichia coli is one of the major pathogenic bacteria that result in the urinary system infection. Various kinds of pathogenic microorganisms are becoming increasingly resistant to antibiotics that commonly used clinically.
