血清SP-A和IL-6水平对新生儿呼吸窘迫综合征病情严重程度及预后的评估价值

摘要

目的:探讨血清肺表面活性蛋白A(surfactant protein A,SP-A)和白细胞介素-6(interleukin-6,IL-6)水平对新生儿呼吸窘迫综合征(neonatal respiratory distress syndrome,NRDS)病情严重程度及预后的评估价值。方法:选择2019年3月至2021年3月海南省妇女儿童医学中心接诊的75例NRDS患儿作为观察组,并选择同期分娩的70例无NRDS的早产儿作为对照组。比较对照组和观察组,以及观察组不同病情程度、不同预后患儿血清SP-A、IL-6水平,并采用logistic回归分析法分析血清SP-A和IL-6与NRDS患儿预后的关系。结果:观察组患儿的血清SP-A和IL-6水平均明显高于对照组,差异均有统计学意义(均P<0.05);观察组不同病情程度患儿血清SP-A和IL-6水平比较,差异均有统计学意义(均P<0.05);观察组的死亡患儿的血清SP-A、IL-6水平明显高于存活组,差异均有统计学意义(均P<0.05);经logistic回归分析显示:血清SP-A和IL-6均是影响NRDS患儿预后的危险因素(均P<0.05)。结论:血清SP-A和IL-6水平有助于评估NRDS患儿病情严重程度及预后,临床应用价值高。

Evaluation value of serum SP-A and IL-6 levels on severity and prognosis of neonatal respiratory distress syndrome

Abstract

Objective: To study the evaluation value of serum pulmonary surfactant protein A (SP-A) and interleukin-6 (IL-6) levels on severity and prognosis of neonatal respiratory distress syndrome (NRDS). Methods: Seventy-five children with NRDS admitted in our hospital from March 2019 to March 2021 were selected as the observation group, and 70 premature infants without NRDS delivered in our hospital in the same period were selected as the control group. The serum SP-A and IL-6 levels between the observation group and the control group, and the observation group children with different degree disease and prognosis were compared, and Logistic regression analysis was used to analyze the relationship between serum SP-A and IL-6 and the prognosis of children with NRDS. Results: The serum SP-A and IL-6 levels in the observation group children were significantly higher than those in the control group (all P<0.05); serum SP-A and IL-6 levels in children with different disease degrees in the observation group was statistically significant (all P<0.05); the serum SP-A and IL-6 levels in dead children of the observation group were significantly higher than those in the survival children, with statistical significance (P<0.05); the logistic regression analysis showed that serum SP-A and IL-6 were risk factors affecting the prognosis of NRDS (P<0.05). Conclusion: Serum SP-A and IL-6 levels are helpful to evaluate the severity and prognosis of patients with NRDS, and have high clinical application value.
