定向技术辅助下经脑沟入路显微切除术治疗幕上浅表低级别胶质瘤的疗效

摘要

目的:探究定向技术辅助下经脑沟入路显微切除术治疗幕上浅表低级别胶质瘤的疗效及对神经功能恢复、应激反应的影响。方法:回顾性分析2018年10月至2020年10月在中国人民解放军陆军第七十三集团军医院进行治疗的100例幕上浅表低级别胶质瘤患者的临床资料,根据手术方式的不同分为对照组与观察组,每组50例。对照组进行传统经脑沟入路显微切除手术,观察组进行定向技术辅助下经脑沟入路显微切除手术。对比2组肿瘤切除情况及手术前、手术后2周的美国国立卫生研究院卒中量表(National Institutes of Health Stroke Scale,NIHSS)评分和日常生活能力(Activities of Daily Living,ADL)评分;检测并比较2组不同时间点[术前1 d(T0)、术后1 d(T1)、术后3 d(T2)]的心率(heart rate,HR)、平均动脉压(mean arterial pressure,MAP)、皮质醇(corticosteroids,Cor)、促肾上腺皮质激素(adrenocorticotrophic hormone,ACTH)水平。结果:观察组有效切除率为96.00%,高于对照组的82.00%(P<0.05);手术后,观察组NIHSS评分低于对照组(P<0.05),ADL评分高于对照组(P<0.05);观察组T1、T2时的HR、MAP及血清Cor、ACTH水平均低于对照组(均P<0.05)。结论:定向技术辅助下经脑沟入路显微切除术治疗幕上浅表低级别胶质瘤具有较高的肿瘤有效切除率,可降低患者神经功能损伤,提高自理能力,减轻手术应激反应。

Effect of microsurgery via sulcus approach assisted by directional technique in the treatment of supratentorial superficial low-grade gliomas

Abstract

Objective: To explore the efficacy of microsurgery via sulcus approach assisted by directional technique in the treatment of supratentorial superficial low-grade gliomas and its influence on nerve function recovery and stress response. Methods: A total of 100 patients with supratentorial superficial low-grade glioma treated in our hospital from October 2018 to October 2020 were selected and divided into a control group and an observation group, with 50 cases in each group. The control group was treated with traditional microsurgery via sulcus approach, and the observation group was treated with microsurgery via sulcus approach assisted by directional technique. The National Institutes of Health Stroke Scale (NIHSS) score and activities of daily living (ADL) score before and 2 weeks after the surgery were compared between the 2 groups. The levels of heart rate (HR), mean arterial pressure (MAP), corticosteroids (Cor) and adrenocorticotropic hormone (ACTH) were detected and compared between the 2 groups at different time points [1 day before the operation (T0), 1 day after the operation (T1) and 3 days after the operation (T2)]. Results: The effective resection rate of the observation group was 96.00%, which was higher than 82.00% of the control group (P<0.05). After the operation, the NIHSS score in the observation group was lower than that in the control group (P<0.05), and the ADL score was higher than that in the control group (P<0.05). The HR, MAP, serum Cor and ACTH levels of T1 and T2 in the observation group were lower than those in the control group (P<0.05). The levels of HR, MAP and serum Cor, ACTH at T1 and T2 in the observation group were lower than those in the control group (P<0.05). Conclusion: Microsurgery via sulcus approach assisted by directional technique has a high effective tumor resection rate in the treatment of supratentorial superficial low-grade gliomas, which can reduce the neurological damage, improve self-care ability and reduce surgical stress response.
