高压氧治疗对重型颅脑损伤术后患者神经功能及肢体功能的影响

摘要

目的:探讨高压氧治疗对重型颅脑损伤术后患者神经功能及肢体功能的影响。方法:选择首都医科大学附属复兴医院2017年1月至2019年12月收治的92例重型颅脑损伤术后患者为研究对象,随机分为观察组与对照组,每组46例,对照组患者术后给予常规治疗,观察组患者进一步联合应用高压氧治疗,比较两组患者治疗前后美国国立卫生院卒中量表(National Institutes of Health Stroke Scale,NIHSS)评分、Fugl-Meyer运动功能评价量表(Fugl-Meyer Assessment,FMA)评分及血清神经元特异性烯醇化酶(neuron-specific enolase,NSE)、中枢神经特异性蛋白(S100 calcium-binding protein β,S100β)水平的变化。结果:治疗后,NIHSS评分及血清NSE、S100β水平在两组患者均有显著降低,且观察组降低更加明显,差异均有统计学意义(P<0.05);治疗后,上肢、下肢及FMA总分在两组患者均有显著升高,且观察组升高更加明显,差异均有统计学意义(P<0.05)。结论:重型颅脑损伤术后患者应用高压氧治疗,可以有效改善患者的神经功能和肢体功能,促进患者的术后康复。

Abstract

Objective: To investigate the effect of hyperbaric oxygen therapy on neurological function and limb function in patients with severe craniocerebral injury after operation. Methods: Ninety-two patients with severe craniocerebral injury who were treated in Fuxing Hospital Affiliated to Capital Medical University from January 2017 to December 2019 were selected as the research objects. They were randomly divided into observation group (n=46) and control group (n=46). The changes of National Institutes of Health Stroke Scale (NIHSS) score, Fugl-Meyer Assessment (FMA) score and serum neuron-specific enolase (NSE) and S100 calcium-binding protein β (S100β) level were compared between the two groups before and after treatment. Results: After treatment, NIHSS score and serum NSE, S100β level in both groups were significantly decreased, and the decrease in the observation group was more obvious, the differences were of significant (P<0.05). After treatment, the FMA total scores and upper limbs, lower limbs in both groups were significantly increased, and the increase in the observation group was more obvious, and the differences were of significance (P<0.05). Conclusion: The application of hyperbaric oxygen therapy in patients with severe craniocerebral injury can effectively improve the neurological function and limb function of patients, and promote the postoperative rehabilitation of patients.
