声辐射力超声弹性成像技术在狼疮肾炎患者中的应用

摘要

目的:应用声辐射力超声弹性成像技术(acoustic radiation force ultrasound elastography,ARFI)测量狼疮肾炎(lupus nephritis,LN)患者肾实质的弹性,早期评估狼疮肾炎患者肾的损害。方法:对30例狼疮肾炎患者(患病组)及30例健康志愿者(健康组)的左右肾分别进行常规超声及弹性检查,比较分析两组之间的差异。绘制受试者工作特征曲线(receiver operating characteristic curve,ROC)曲线),并计算出敏感性、特异性、阳性预测值及阴性预测值。结果:两组左右肾的弹性值差异无统计学意义(P>0.05)。以右肾为例,两组肾弹性值差异有统计学意义(P<0.05)。根据约登指数算出剪切波传播速度(shear wave velocity,SWV)的截断值,当SWV=3.01 m/s时,狼疮肾炎的ROC的曲线下面积(AUC)为0.898,敏感性83.3%,特异性86.7%,阳性预测值86.2%,阴性预测值83.9%。结论:ARFI为狼疮肾炎患者的早期诊断提供一种新的无创定量评估指标。

Application of acoustic radiation force ultrasound elastography in measurement of kidney elasticity in patients with lupus nephritis

Abstract

Objective: To investigate the early evaluation of renal damage in patients with lupus nephritis (LN) by measuring the elasticity of renal parenchyma in patients with lupus nephritis by acoustic radiation force ultrasound elastography (ARFI). Methods: The left and right kidneys of 30 patients with lupus nephritis (the diseased group) and 30 healthy volunteers (the healthy group) were examined by routine ultrasound and elastic check, and the differences between the two groups were compared and analyzed. Receiver operating curves (ROC) were drawn and sensitivity, specificity, positive predictive value, and negative predictive value were calculated. Results: There was no significant difference in left and right kidney elasticity between the two groups (P>0.05). Taking the right kidney as an example, there was a significant difference in renal elasticity between the diseased group and the healthy group (P<0.05). Taking shear wave velocity (SWV) = 3.01 m/s as the cut-off value to diagnose the lupus nephritis, the area under the curve (AUC) was 0.898, and the sensitivity, specificity, positive predictive value and negative predictive value was 83.3%, 86.7%, 86.2% and 83.9% respectively. Conclusion: ARFI provides a new noninvasive evaluation method for the early diagnosis of lupus nephritis.
