老年髋骨骨折患者主要照顾者预期性悲伤的现状及其影响因素

摘要
目的：描述老年髋骨骨折患者主要照顾者预期性悲伤的现状，探讨其影响因素。方法：采用便利抽样法选择2018年10月至2019年5月南京市某三级甲等医院108名老年髋骨骨折患者的主要照顾者为研究对象，以一般资料调查表，照顾者负担量表(caregiver burden inventory，CBI)和预期性悲伤量表(anticipatory grief scale，AGS)为工具进行调查。结果：老年髋骨骨折患者主要照顾者预期性悲伤得分为(65.52±8.30)分；多元回归分析结果显示，老年髋骨骨折患者主要照顾者的年龄、文化程度、家庭人均月收入、照顾时间、与患者关系、照顾者负担程度、专业人员指导(社区)及患者的自理能力是照顾者预期性悲伤的影响因素(P<0.05)。结论：居家老年髋骨骨折患者的主要照顾者存在中等水平的预期性悲伤，医护人员关注患者心理问题的同时，应将照顾者也列入心理护理对象的范畴，并积极探讨有效的护理措施，促进照顾者的身心健康。

Current status of anticipatory grief among primary family caregivers of elderly patients with hip fracture and its influencing factors

Abstract
Objective: To describe the current status of anticipatory grief among major family caregivers of elderly patients with hip fracture and to explore its influencing factors. Methods: Totally 108 primary family caregivers of elderly patients with hip fracture in a third-class hospital in Nanjing from Oct 2018 to May 2019 were selected by convenience sampling, using general information questionnaire, caregiver burden inventory (CBI) and anticipatory grief scale (AGS) for further investigation. Results: The mean score of AGS was 65.52±8.30. Multiple linear regression analysis showed that age, educational background, monthly income per capita, care times, relationship with patients, caregiver burden, professional guidance (community) and illness condition were the main factors (P<0.05). Conclusion: The primary family caregivers of elderly patients with hip fracture have middle level anticipatory grief. The medical staff should not only pay attention to the mental status of caregivers but rank them as the psychological nursing objects and take effective measures to promote their physical and mental health.
