2型糖尿病患者下肢动脉粥样硬化分级与心脑血管病变的相关性

摘要

目的:分析2型糖尿病患者下肢动脉粥样硬化分级与心脑血管病变的相关性,并探究2型糖尿病患者下肢动脉斑块形成的危险因素。方法:收集2010年1月至2016年12月在合肥市第一人民医院内分泌住院的2型糖尿病患者2 824例,将患者分成有脑梗死组与无脑梗死组、有冠心病组与无冠心病组。并根据下肢动脉彩色多普勒超声结果,按动脉硬化程度进行分级,采用logstic回归分析下肢动脉粥样硬化分级与脑梗死、冠心病的相关性。结果:与下肢动脉粥样硬化1级患者比较,3级(OR=2.423)和4级(OR=2.661)患者发生脑梗死的风险高,3级(OR=1.174)和4级(OR=2.120)患者发生发生冠心病的风险也较高。2型糖尿病患者下肢动脉斑块的形成与年龄(OR=1.089,P<0.001)、收缩压(OR=1.008,P=0.001)、血肌酐(OR=1.007,P<0.001)、低密度脂蛋白胆固醇(OR=1.157,P=0.003)有关。结论:2型糖尿病患者下肢动脉粥样硬化分级越高,发生脑梗死及冠心病的风险越大,早期对下肢动脉斑块形成进行临床干预,有利于预防心脑血管疾病的发生。

Correlation between atherosclerosis grading of lower extremity and cardiovascular and cerebrovascular diseases in type 2 diabetes

Abstract

Objective: To analyze the correlation between lower extremity atherosclerosis grading and cardiovascular and cerebrovascular diseases in patients with type 2 diabetes, and to explore the risk factors of lower extremity arterial plaque formation in type 2 diabetes. Methods: A total of 2 824 patients with type 2 diabetes mellitus who were hospitalized in Hefei First People’s Hospital from January 2010 to December 2016 were enrolled. The patients were divided into the cerebral infarction group and the non-cerebral infarction group, including the coronary heart disease group and the non-coronary heart disease group. According to the results of color Doppler ultrasound of lower extremity arteries, the degree of arteriosclerosis was graded. Logistic regression was used to analyze the correlation between lower extremity atherosclerosis grading and cerebral infarction and coronary heart disease. Results: Patients with grade 3 (OR=2.423) and grade 4 (OR=2.661) had a higher risk of cerebral infarction than grade 3 (OR=1.174) and grade 4 (OR=2.120). The risk of developing coronary heart disease is also high in patients. Formation and age of lower extremity arterial plaque in type 2 diabetes (OR=1.089, P<0.001), systolic blood pressure (OR=1.008, P=0.001), serum creatinine (OR=1.007, P<0.001), low-density lipoprotein cholesterol (OR=1.157, P=0.003) related. Conclusion: The higher the classification of lower extremity atherosclerosis in patients with type 2 diabetes, the greater the risk of cerebral infarction and coronary heart disease. Early clinical intervention on the formation of lower extremity arterial plaque is beneficial to prevent cardiovascular and cerebrovascular diseases.
