妊娠合并肾盂肾炎68例的临床分析

摘要
目的:探究分析妊娠合并肾盂肾炎患者的临床情况。方法:纳入2018年1月至2019年9月北京市通州区妇幼保健院门诊留院观察的妊娠合并肾盂肾炎孕妇68例,分析其临床症状表现情况,并进行血常规、尿常规检查,尿细菌培养,分析病原菌种类及占比,并以临床常用抗生素药物进行药敏实验。结果:妊娠合并肾盂肾炎患者常见症状表现以发热、头痛、寒颤等为主,且多数患者伴有尿频尿急、恶心呕吐。在68例孕妇中,53例孕妇存在贫血,占比77.94%;59例孕妇存在尿液白细胞值偏高,占比86.76%。共分离出病原菌49株,以大肠埃希菌占比最高。药敏结果显示:头孢菌素类药物敏感性较好,耐药性低,而对氨苄西林、左氧氟沙星等药物的耐药性较强。结论:针对妊娠合并肾盂肾炎孕妇的治疗,应结合菌株培养结果及药敏结果合理选择抗菌药物,从而提高治疗效果。

Clinical analysis of 68 cases of pregnancy with pyelonephritis

Abstract
Objective: To analyze the clinical situation of pregnancy with pyelonephritis. Methods: Sixty-eight pregnant women with pyelonephritis during pregnancy were included in the Outpatient Clinic of Tongzhou Maternal and Child Health Hospital from January 2018 to September 2019. Their clinical symptoms were analyzed. Routine blood and urine tests, urine bacterial culture, types and proportion of pathogenic bacteria were carried out, and drug sensitivity tests were carried out with commonly used antibiotics. Results: The main symptoms of pregnancy patients with pyelonephritis were fever, headache and shiver, and most of them were accompanied with frequent urination, nausea and vomiting. Among the 68 pregnant women, 53 had anemia, accounting for 77.94%, and 59 had high urine leukocyte value, accounting for 86.76%. A total of 49 pathogenic bacteria were isolated, especially Escherichia coli. The results of drug sensitivity showed that it was sensitive to cephalosporins and had low drug resistance, while it was resistant to ampicillin, levofloxacin and other drugs. Conclusion: During the actual treatment of pregnant women with pyelonephritis, we should combine the results of strain culture and drug sensitivity results to select the antibiotics reasonably, so as to improve the treatment effect.
