男性阻塞性睡眠呼吸暂停低通气综合征患者睡眠时体位及翻身频率的特点及作用

摘要

目的:探讨不同严重程度男性阻塞性睡眠呼吸暂停综合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)患者睡眠时体位及夜间翻身频率的特点并分析其在OSAHS疾病发生发展过程中的作用。方法:回顾性分析行多导睡眠监测的男性患者,在剔除翻身频率的离群值后,共计184例患者。根据呼吸暂停低通气指数(apnea-hypopnea index,AHI)分为正常组(n=22)和轻度组(n=25)、中度组(n=29)及重度组(n=108),并根据多导睡眠监测的结果对4组患者的夜间体位,翻身频率以及一些基本信息进行统计学分析。结果:4组研究对象BMI差异有统计学意义(P<0.05);仰卧位时AHI,心率明显大于非仰卧位时,非仰卧位中人们更偏向于右侧卧位。患者睡眠时仰卧位时间与翻身频率呈负相关(r=0.579,P<0.01)。其中轻度组患者翻身频率最高但仰卧位时间最少。结论:OSAHS患者严重程度与肥胖密切相关,取侧卧位比取仰卧位可有效增加血氧饱和度,降低心率与不良呼吸阻塞事件。夜间翻身次数的改变可能在患者长期疾病发展过程中起重要作用。部分患者在长期打鼾憋气下可能以增加夜间翻身频率来降低睡眠时呼吸阻塞事件的发生,尤其以轻度OSAHS患者表现较为明显。

Characteristics and effect of body position and turnover frequency in male obstructive sleep apnea-hypopnea syndrome patients

Abstract

Objective: To investigate the characteristics of sleep posture and turnover frequency in male obstructive sleep apnea-hypopnea syndrome (OSAHS) patients with different severity and to analyze its role in the development of OSAHS. Methods: A retrospective analysis of male patients undergoing polysomnography was performed in 184 patients after removal of outliers. According to AHI, there were 22 cases in the normal group, 25 cases in the mild group, 29 cases in the moderate group, and 108 cases in the severe group. According to the results of polysomnography, a statistical analysis was made of the night posture, turnover frequency and some basic information of 4 groups. Results: The BMI of the 4 groups had statistical differences. People who in the supine position that the AHI and heart rate were significantly higher than subjects in non-supine position. subjects in the non-supine position were more inclined to the right lateral position. The time between the supine position and the turnover frequency during sleep was statistically significant (r=0.579, P<0.01). Among them, the mild group had the highest turnover frequency but the least time in the supine position. Conclusion: The severity of OSAHS patients is closely related to obesity. Taking the lateral position rather than the supine position can effectively increase blood oxygen saturation and reduce heart rate and adverse respiratory events. Changes in the number of turn over during sleep at night may play an important role in the patient’s long-term disease development. Some patients may increase the turnover frequency during long-term snoring to reduce the occurrence of respiratory obstruction during sleep, especially in patients with mild OSAHS.
