左旋肉碱在运动发育迟缓患儿中的疗效

摘要
目的:探讨左旋肉碱对运动发育迟缓患儿的治疗作用。方法:选取诊断为运动发育迟缓的患儿40例,随机分为治疗组与对照组(各20例),对照组采用运动学分析评估及康复训练方法,治疗组在此基础上口服左旋肉碱[50 mg/(kg·d−1)]。治疗前及治疗3个月后,采用串联质谱分析检测游离肉碱含量,分别采用表面肌电分析评估系统、Peabody运动发育评定量表粗大运动部分及精细运动部分评定肌张力、粗大及精细运动能力。结果:治疗前,两组游离肉碱均偏低,治疗3个月后,治疗组游离肉碱均有所提升,对照组仍偏低;两组治疗后表面肌电分析评估系统中的肌电信号时域范围积分肌电值(integrated electromyogram,iEMG)、粗大及精细运动功能评分较前均有所提高,治疗组优于对照组;治疗组iEMG、粗大及精细运动功能评分与对照组的差异有统计学意义(均P<0.05)。结论:口服左旋肉碱能进一步提高运动发育迟缓患者的肌张力、粗大及精细运动功能。

Therapeutic effect of L-carnitine in children with motor retardation

Abstract
Objective: To investigate the therapeutic effect of L-carnitine on children with motor retardation. Methods: Forty children with diagnosis of motor retardation were randomly divided into a treatment group and a control group. The control group was evaluated by kinematic analysis and rehabilitation. The treatment group was given oral L-carnitine[50 mg/(kg·d−1)]. Before treatment and after 3 months of treatment, the content of free carnitine was detected by tandem mass spectrometry. The surface electromyography evaluation system, the gross motion part of the Peabody exercise development rating scale and the fine motor part were used to evaluate muscle tension, gross and fine motor ability. Results: Before treatment, the free carnitine levels in the two groups were low. After 3 months of treatment, the free carnitine in the treatment group was improved, and the control group was still low. The electromyographic signals in the system were evaluated by the surface electromyography after treatment. The time domain range myoelectric integral value (iEMG), gross and fine motor function scores were improved earlier than the control group (P<0.05). The iEMG, gross and fine motor function scores of the treatment group were compared with the control group. There are significant differences. Conclusion: L-carnitine can improve muscle tension, gross and fine motor function in patients with motor retardation.
