盆腔弥漫性子宫内膜异位病灶恶变为低级别子宫内膜间质肿瘤进一步转化为未分化子宫肉瘤1例

摘要
子宫内膜异位症(endometriosis，Ems)恶变为子宫内膜间质肿瘤(endometrial stromal sarcoma，ESS)较为罕见，由盆腔外弥漫性Ems病灶恶变为低级别ESS(low-grade endometrial stromal sarcoma，LGESS)进一步转化为未分化子宫肉瘤(undifferentiated uterine sarcoma，UUS)在国内外更是鲜有报道。本例为42岁女性患者，初次就诊时术后病理诊断为Ems。17个月后复发，再次手术，术后病理诊断为左卵巢低级别ESS，Ems症。1年后再次复发，病情进展为未分化ESS。术后予以放射、化学药物治疗，均未见明显疗效，9个月后患者去世。

Abstract
The incidence of malignant transformation of endometriosis (Ems) into endometrial stromal tumors is very low. The malignant transformation from extrapelvic diffuse Ems to low-grade endometrial stromal sarcoma (LGESS) to undifferentiated uterine sarcoma (UUS) is rarely reported. This paper reports a 42-year-old woman with endometriosis in Shanghai general hospital at the first time. Seventeen months later, the patient relapsed, with postoperative pathology: LGESS of the left ovary, endometriosis. After 1 year, the patient relapsed and developed undifferentiated endometrial stromal sarcoma. There was no significant effect of radiotherapy and chemotherapy after operation. Nine months later, the patient died of cachexia.
