蛆虫清创治疗联合血管腔内成形术对糖尿病足创面的疗效

摘要
目的:观察蛆虫清创治疗联合血管腔内成形术治疗糖尿病足的疗效。方法:选取2013年1月至2015年12月解放军第四五四医院内分泌科收治的缺血性糖尿病足患者,应用踝肱指数(ankle-brachial ratio,ABI)、多层螺旋CT血管成像(CT angiography,CTA)等评估周围血管病变,用Wagner分级系统评估创面情况,将患者随机分成2组,试验组接受蛆虫清创治疗(maggot debridement therapy,MDT)联合腔内血管成形术(percutaneous transluminal angioplasty,PTA)治疗,对照组仅接受PTA治疗。结果:PTA术后患者疼痛症状、冷感、间歇性跛行均有不同程度的改善,患者ABI较术前提高明显(P<0.05);试验组较对照组创面愈合时间明显缩短(P<0.05)。结论:PTA是治疗下肢缺血性疾病的有效方法,MDT联合PTA能明显缩短糖尿病足创面的愈合时间。

Effect of maggot debridement therapy combined with endovascular angioplasty on diabetic foot

Abstract
Objective: To observe the therapeutic effect of maggot debridement therapy combined with endovascular angioplasty in the treatment of diabetic foot. Methods: We collected patients with ischemic diabetic foot ulcers in Department of Endocrinology in The 454th Hospital of PLA from January 2013 to December 2015. The peripheral vascular disease was assessed by ankle-brachial index (ABI) and multi-slice spiral CT angiography. Wagner grading system was used to assess the wound grade. The patients were randomly divided into 2 groups: the experiment group received either maggot debridement therapy (MDT) or percutaneous transluminal angioplasty (PTA), while the control group received only PTA. Results: All patients who were with pain, cold, intermittent claudication had different degrees of improvement after surgery, and ABI increased significantly (P<0.05); patient who were treated with MDT, wound healing time was significantly shortened (P<0.05). Conclusion: PTA was an effective method for the treatment of ischemic diseases of the lower extremities. MDT combined with PTA could significantly shorten the healing time of diabetic foot wounds.
