细胞核因子κB 受体活化因子在宫颈鳞癌中的表达及其与 临床病理因素和预后的关系

摘要

目的:探讨细胞核因子κB受体活化因子(receptor activator of nuclear factor-kappa B,RANK)在宫颈 鳞癌中的表达及其与临床病理因素和预后的关系。方法:回顾性分析2012年11月至2015年5月在 华中科技大学同济医学院附属协和医院西院妇产科行手术治疗的57例宫颈鳞癌患者,收集其病理 及随访资料,采用免疫组织化学法检测宫颈鳞癌及癌旁组织中RANK的表达情况,分析RANK表 达与临床病理因素的关系,采用单因素及多因素分析影响宫颈鳞癌患者总生存率及累计复发率的 预后因素。结果:肿瘤组织中RANK表达较癌旁组织高(7.263±2.349 vs 5.018±2.303,P<0.001), 且RANK高表达与国际妇产科联合会(International Federation of Gynecology and Obstetrics,FIGO) 分期(P=0.047)、淋巴结转移(P=0.011)、淋巴脉管浸润(lymph vascular space invasion,LVSI) (P=0.002)相关;与RNAK低表达组比,RNAK高表达组5年总生存率更低(32.14% vs 79.31%, P=0.036),术后5年累计复发率更高(64.29% vs 17.24%,P=0.032);单因素分析示FIGO分期、淋 巴结转移及RANK高表达为影响患者总生存率和累计复发率的预后因素(P<0.05)。多因素分析示 FIGO分期及RANK高表达为影响总生存率和累计复发率的预后因素(P<0.05)。结论:RANK在宫 颈鳞癌中高表达,RANK高表达与宫颈癌恶性临床病理因素相关,RANK高表达提示宫颈癌预后 不良。

Expression of receptor activator of nuclear factor-kappa B in cervical squamous cell carcinoma and its correlation with clinicopathological factors and prognosis

Abstract

Objective: To investigate the expression of receptor activator of nuclear factor-kappa B (RANK) in cervical squamous cell carcinoma, and analyze its clinical significance and prognosis. Methods: Fifty-seven patients with cervical squamous cell carcinoma patients who underwent curative resection between November 2012 and May 2015 in our hospital was retrospectively analysed. Immunohistochemistry was performed to determine the expression of Rank in tumor and paratumor tissue. The connection between RANK expression and pathological characteristics and clinical feature was analysed. Also, prognostic factors of overall survival and cumulative recurrence rate were analyzed by univariate and multi-logistic regression analysis. Results: RANK protein expression was significantly higher in tumour tissues (7.263±2.349 vs 5.018±2.303, P<0.001) and a high expression of RANK was significantly correlated with FIGO-stage (P=0.047), lymph node metastasis (P=0.011), and lymph vascular space invasion (P=0.002). Patients with high expression of RANK had a significantly lower 5-year overall survival rate (32.14% vs 79.31%, P=0.036) and higher cumulative recurrence rate (64.29% vs 17.24%, P=0.032) than those with low expression. FIGO-stage, lymph node metastasis and RANK staining was prognostic factors of overall survival and cumulative recurrence rate by univariate analysis (P<0.05). FIGO-stage and RANK staining was independent risk factor of overall survival and cumulative recurrence rate by multi-logistic regression analysis (P<0.05). Conclusion: RANK expression is higher in tumor than in paratumor tissues. Overexpression of RANK is closely related to invasion and metastasis. Overexpression of RANK protein suggests poor progress in cervical squamous cell carcinoma.
