To analyze the problems of pathological autopsy report documents and to explore the importance and necessity of unified and specification pathological autopsy report and its basic formats, the 16 pathological autopsy reports from 6 provinces and cities in recent years were retrospectively study and the quality problems of these documents were analyzed. Some problems were found in the pathological autopsy report: there was not unified document’s name and basic formats, the records of autopsy and slices observation were incomplete, some of the pathological description and diagnosis were inaccurate and not specification, there were not death analysis and conclusion of cause of death in the most pathological reports, there were a lot of problems such as printing, typesetting, wrong character and pictures. Suggestions for improvement: The relevant departments of our country and Chinese Pathological Association should pay attention to and strengthen the works of unified and standardized pathological autopsy report documents. The name of the document could be “The Pathological Autopsy (Examination) Report for XXX Department (Unit)”. The basic format of document includes the basic information, the basic case, summary of data, inspection process (autopsy or pathological examination), analysis, inspection opinion (conclusion) and attachments. In the context of the rule of law and the scientific and standardized requirements for the professional work, it is not only important, but also very necessary to unify and standardize the mane and its basic format of the pathological autopsy document. The basic format of the first pathological report document here is scientific, standardized, practical and feasible.
