农村先天性心脏病儿童父母疾病不确定感与健康信息素养及心理弹性的相关性

摘要
目的:调查农村先天性心脏病(congenital heart disease,CHD)儿童父母疾病不确定感的临床现状,并探讨健康信息素养(health information literacy,HIL)及心理弹性与父母疾病不确定感的相关性。方法:采用HIL调查表、父母疾病不确定感量表(Parents’ Perception Uncertainty in Illness Scale,PPUS-FM)对125名农村CHD儿童父母进行调查,数据结果应用SPSS22.0软件进行统计分析。结果:农村CHD儿童父母的疾病不确定感得分为102.44±21.91,87.2%的患者得分在中等以上水平,不同家庭平均月收入、文化程度、儿童年龄、儿童独生子女情况的疾病不确定感得分分布差异具有统计学意义(P<0.05);HIL的得分为88.24±7.69,处于中等水平;心理弹性的得分为69.56±6.93,总分及各维度得分均明显低于国内常模(P<0.05);Pearson相关性分析结果显示,除信息道德维度以外,HIL及心理弹性的各维度得分均与疾病不确定感总分及各维度得分呈负相关关系(r=−0.257~−0.179,P<0.05)。结论:农村CHD儿童父母的疾病不确定感较高,HIL及心理弹性与其疾病不确定感明显相关,HIL及心理弹性越好则其疾病不确定感越低。临床医护人员应注重HIL的培养及内在积极心理资源的调动,以降低疾病不确定感。

Abstract
Objectives: To investigate the current clinical situations of uncertainty in illness of parents with congenital heart disease (CHD) children in rural areas, and to discuss the correlation of health information literacy (HIL) with mental elasticity and uncertainty in illness. Methods: Health Information Literacy Questionnaire and Parents’ Perception Uncertainty in Illness Scale (PPUS-FM) were adopted to investigate into 125 parents of CHD children in rural areas. Statistical analysis of the data obtained was conducted with SPSS22.0 software. Results: The score of uncertainty in illness of the parents of CHD children in rural areas was 102.44±21.91; the scores of 87.2% of the patients were in the medium and above level. Differences in distribution of scores of uncertainty in illness due to different average household monthly income, educational level, children’s ages, children as only child or not were of statistical significance (P<0.05). The score of HIL was 88.24±7.69, which was in the middle level. The score of mental elasticity was 69.56±6.93; the total score and scores of various dimensions were evidently lower than the national norms (P<0.05). As indicated by the results of Pearson correlation analysis, scores of all dimensions except for information morality dimension of HIL and mental elasticity were negatively correlated with the total score and scores of all dimensions of uncertainty in illness (r=−0.257~−0.179, P<0.05). Conclusion: Uncertainty in illness of the parents of CHD children in rural areas is high. HIL and mental elasticity are evidently correlated with uncertainty in illness; when HIL and mental elasticity are better, the uncertainty in illness is lower. Clinical medical care personnel should cultivate HIL and mobilize internal positive psychological resources to reduce uncertainty in illness.
