慢性肾病患者的限盐饮食意愿及团体护理模式的应用

摘要
目的:研究慢性肾病(chronic kidney disease,CKD)患者限盐饮食意愿调查及团体护理模式的应用价值。方法:前瞻性研究济南军区总医院180例CKD患者临床病历资料,根据健康信念模式编制限盐饮食意愿调查表,观察患者对限盐饮食的意愿和依从性,分析影响患者限盐饮食意愿的相关因素。根据调查结果开展团体活动护理,探讨团体护理模式对改善患者限盐饮食意愿的应用价值。结果:单因素和多因素分析结果显示文化程度、经济收入、专业知识水平及自觉症状是影响患者限盐饮食意愿的独立因素(P<0.05)。护理后CKD患者限盐饮食具有良好依从性的占83.33%,显著高于护理前(P<0.05)。结论:CKD患者存在限盐饮食意愿偏低、依从性差的问题,开展团体护理有助于改善患者限盐饮食意愿,提高治疗依从性,具有较高临床应用价值。

Abstract
Objective: To investigate the willingness to salt-restriction diet and the application value of group nursing model in patients with chronic kidney disease (CKD). Methods: The medical records of 180 patients with CKD in our hospital were prospectively studied. According to the health belief model, questionnaires of willingness to salt-restriction diet were designed. The willingness and compliance of patients to salt-restriction diet were observed, and the related factors influencing the patients’ willingness to salt-restriction diet were analyzed. According to the results, group activities were carried out to explore the application value of group nursing model in improving patients’ willingness to salt-restriction diet. Results: Univariate and multivariate analysis showed that education level, income, professional knowledge level and conscious symptoms were the independent factors influencing the patients’ willingness to salt-restriction diet (P<0.05). After nursing, the proportion of patients with good compliance in patients with CKD (83.33%) was significantly higher than that before nursing (P<0.05). Conclusion: There are problems such as low willingness to salt-restriction diet and poor compliance in patients with CKD. The implementation of group nursing is helpful to improve patients' willingness and compliance to salt-restriction diet.
