摘要

目的:探讨影响二次放疗食管癌预后的因素。方法:14例食管癌放疗后复发患者接受二次放疗。给予三维适形放疗,分别采用单因素和多因素风险模型进行COX风险比例分析,最后采用Kaplan-Meier生存分析和Log-Rank检验分析影响二次放疗食管癌的预后因素。结果:单因素分析显示肿瘤原发部位是影响预后的因素;多因素分析显示肿瘤长度和分化程度是影响生存时间的主要因素;Kaplan-Meier生存分析和Log-Rank检验分析显示影响二次放疗食管癌患者生存率的危险因素是肿瘤分化程度,高分化患者生存时间明显优于中低分化。结论:分化程度是影响二次放疗食管癌的主要预后因素。

Prognostic factors of esophageal cancer in patients with the secondary radiotherapy

Abstract

Objective: To investigate the prognostic factors in patients with esophageal cancer receiving the secondary radiotherapy. Methods: Fourteen patients with esophageal cancer after radiotherapy were treated with the secondary radiotherapy. The univariate factor and multivariate factor risk analysis model were used in analyzing COX risk ratio, and Kaplan-Meier survival analysis and Log-Rank test analysis were used to identify the prognostic factor in the secondary radiotherapy treatment on esophageal carcinoma. Results: The results of univariate analysis showed that the primary tumor site was a prognostic factor, multivariate analysis showed that the length and tumor differentiation were the main factor of affecting the survival time, Kaplan-Meier survival analysis and Log-Rank test analysis showed that tumor differentiation was the risk factor in the secondary radiotherapy treatment on esophageal cancer, the overall survival time of patients with high tumor differentiation were significantly better than those of the low tumor differentiation. Conclusion: Tumor differentiation was a significant prognostic factor in the outcome in the patients receiving the secondary radiotherapy.
