家庭肠内营养及其护理干预对炎性肠病患者人体成分变化的影响

摘要

目的:了解家庭肠内营养(home enteral nutrition,HEN)支持治疗对炎性肠病(inflammatory bowel disease,IBD)患者人体成份变化的影响。方法:前瞻性选取2013年11月至2014年11月间南京军区南京总医院收治的82例炎症性肠病患者,于出院后24~48 h开始对其实施HEN及相关护理,由营养支持小组(nutrition support team,NST)于3个月后随访并利用生物电阻抗分析(bioelectrical impedance analysis,BIA)法进行人体成分分析,观察比较患者接受营养支持后骨骼肌、体脂肪、蛋白质、体重、体重指数(body mass index,BMI)等指标的变化。结果:经过3个月的HEN支持治疗后,炎症性肠病患者的骨骼肌含量、体脂肪含量、蛋白质含量、体重、BMI、上臂围度、上臂肌肉围度,体内细胞量和基础代谢率明显较HEN治疗前高(P<0.05),而治疗后患者细胞内液和细胞外液较治疗前有所升高,但差异无统计学意义(P>0.05)。结论:经过3个月的HEN支持后,IBD 患者的人体成分各项指标较治疗前明显改善。

Home enteral nutrition and nursing intervention on the impact of changes in body composition in patients with inflammatory bowel disease

Abstract

Objective: Home enteral nutrition (HEN) support therapy on the impact of changes in body composition in patients with inflammatory bowel disease (IBD). Methods: Prospective from November 2013 to November 2014 Nanjing General Hospital of Nanjing Military Command of the 82 cases of IBD, started in 24~48 hours after discharge from hospital to HEN and nursing care of its implementation, from nutrition support group at 3 months follow-up and analysis of body composition using bioelectrical impedance analysis (BIA), observation after receiving nutritional support in patients with skeletal muscle, weight, body mass index (BMI), body fat, protein and other indicators of change. Results: After 3 months of family intestinal within nutrition support treatment, inflammation intestinal disease patients of bones muscle content, body fat content, protein content, weight BMI, arm surrounding degrees, arm muscle surrounding degrees, body cell volume and basal metabolic rate significantly higher than that of HEN treatment (P<0.05), and after treatment patients cell within liquid and cell outside liquid increased more before treatment, but differences no statistics meaning (P>0.05). Conclusion: After 3 months of HEN support, body composition in patients with IBD indicators significantly improved compared with before treatment.
