I型子宫内膜癌中HIF-1和E-cad的表达及其相关性

摘要

目的:探讨在I型子宫内膜癌(endometrial carcinoma,EC)中缺氧诱导因子-1(hypoxia-inducible factor 1,HIF-1)与黏钙蛋白E(E-cadherin)的表达及其与临床病理特征的关系。方法:选择2009年5月至2019年10月山西医科大学第一临床医学院病理确诊的92例I型子宫内膜腺癌组织,60例同期的非典型增生子宫内膜组织以及60例正常的子宫内膜组织,采用链霉亲和素-过氧化物酶-生物素(immunohistochemical staining,SP)免疫组织化学法检测HIF-1与E-cad在组织中的表达,分析其与患者临床病理特征的关系及二者之间的相关性。结果:HIF-1在内膜腺癌组织和非典型增生子宫内膜组织中的阳性表达率分别为73.9%和37.1%,显著高于正常子宫内膜组织中的10.0%,差异有统计学意义(P<0.01);E-cad在内膜腺癌组的表达率为33.7%,低于非典型增生组的56.5%和正常子宫内膜组的93.3%,差异有统计学意义(P<0.01)。内膜腺癌组织中HIF-1呈高表达而E-cad呈低表达,二者呈负相关关系(r=–0.572,P<0.01)。HIF-1在I型EC组织中的表达与患者的年龄和组织学分级无关(P>0.05),E-cad的表达与患者的年龄无关(P>0.05),FIGO分期高、分化程度低、浸润程度深、发生淋巴结转移的I型EC患者HIF-1表达阳性率显著升高(P<0.05),E-cad表达阳性率显著降低 (P<0.05)。结论:HIF-1的高表达和E-cad表达的下调在子宫内膜癌的发生和发展中起重要作用,二者的联合检测对I型EC的诊断及侵袭性判断具有重要意义。

Expression and correlation of HIF-1 and E-cad in type I endometrial carcinoma

Abstract

Objective: To investigate the expression of hypoxia inducible factor-1 (HIF-1) and E-cadherin in endometrial carcinoma and their correlation with clinicopathological features. Methods: Tissues of 92 cases of type I endometrial adenocarcinoma, 60 cases of atypical endometrial hyperplasia and 60 cases of normal endometrial who were treated in the Hospital of Shanxi Medical University from May 2009 to October 2019 were randomly selected. The expression of HIF-1 and E-cadherin were detected by immunohistochemistry and compared with cancerous tissues, atypical hyperplasia tissues and normal tissues, and the correlation between HIF-1 and E-cadherin and the clinicopathological features of type I endometrial cancer were analyzed. Results: The positive expression rate of HIF-1 in the endometrial adenocarcinoma and atypical hyperplasia was 73.9% and 37.1%, respectively, which was significantly higher than 10% of the normal endometrium with statisticly significant differences (P<0.01); the expression rate of E-cad in endometrial adenocarcinoma was 33.7%, which was lower than 56.5% of the atypical hyperplasia and 93.3% of the normal endometrium, which has significant differences (P<0.01). (P<0.01). HIF-1 was highly expressed in endometrial adenocarcinoma, while E-cad was low and there was a negative correlation between them (r=−0.572, P<0.01). The expression of HIF-1 in type I EC was not related to the age and histological grade of the patients (P>0.05). The expression of E-cad was not related to the age of the patients (P>0.05). Conclusion: the high expression of HIF-1 and the down-regulation of E-cad play an important role in the occurrence and development of endometrial carcinoma. The combined detection of HIF-1 and E-cad plays an important role in the diagnosis of type I EC and the determination of its invasiveness.
