摘要

目的:探讨急性心肌梗死(acute myocardial infarction,AMI)患者血清脂蛋白相关磷脂酶A2(lipoprotein associated phospholipase A2,Lp-PLA2)与左室射血分数(left ventricular ejection fraction,LVEF)的关系。方法:纳入2017年6月至2018年12月西安交通大学第一附属医院心内科确诊的816例AMI患者,收集其一般临床资料、血脂、D-二聚体、高敏性C-反应蛋白(high-sensitivity C-reactive protein,hs-CRP)、Lp-PLA2水平及心脏超声心脏射血分数(left ventricular ejection fraction,LVEF)等进行数据,采用Spearman相关分析及多因素线性回归分析方法分析血清脂蛋白相关磷脂酶A2水平与AMI LVEF的相关性。结果:LVEF<50%组AMI患者血清Lp-PLA2水平显著高于LVEF≥50%的患者[(163.05±73.95) ng/mL vs (151.18±62.02) ng/mL,P=0.027)]。经Spearman相关分析,AMI患者的Lp-PLA2水平与LVEF呈负相关(r=−0.140,P<0.05)。多因素线性回归分析显示,Lp-PLA2水平(β=−0.016,P=0.013)、D-二聚体(β=−0.855,P=0.032)、hs-CRP(β=−0.343,P<0.001)与LVEF呈负相关。结论:AMI患者血清Lp-PLA2水平与LVEF呈负相关,血清Lp-PLA2随LVEF的降低而升高。

Abstract

Objective: To investigate the relationship between serum lipoprotein associated phospholipase A2 (Lp-PLA2) level and left ventricular ejection fraction in patients with acute myocardial infarction (AMI). Methods: A total of 816 patients diagnosed with AMI who had undergone coronary angiography in our hospital from June 2017 to December 2018 were enrolled into this study and to be analyzed retrospectively. The serum Lp-PLA2 concentration was detected by ELISA analyzer and the left ventricular ejection fraction (LVEF) was measured by echocardiography. The relationship between serum Lp-PLA2 and left ventricular ejection fraction in acute myocardial infarction was analyzed by spearman analysis and multivariate regression analysis. Results: The expression of serum Lp-PLA2 in patients with LVEF <50% were significantly higher than those in patients with LVEF ≥50% [(163.05±73.95) ng/mL vs (151.18±62.02) ng/mL, P=0.027]. The Spearman correlation analysis showed that the levels of serum Lp-PLA2 were significantly negatively correlated with LVEF (r=−0.140, P<0.05). Multivariate regression analysis showed that Lp-PLA2 (β=−0.016, P=0.013), D-dimer(β=−0.855, P=0.032) and hs-CRP(β=−0.343, P<0.001) were negatively correlated with LVEF. Conclusion: The levels of serum Lp-PLA2 were negatively associated with the left ventricular ejection fraction in patients with acute myocardial infarction. The levels of serum Lp-PLA2 in patients with AMI increase with the decrease of left ventricular ejection fraction.
