SOX9 在甲状腺癌中的表达及其与临床病理特征和预后的关系

摘要

目的:探讨性别决定区Y框蛋白9(sex-determining region Y-box protein 9,SOX9)在甲状腺癌中的表达及其与患者临床病理特征及预后的关系。方法:回顾性分析2015年8月至2017年12月南阳市中心医院收治的98例甲状腺癌患者,收集患者根治术治疗过程中切除的甲状腺癌组织及癌旁组织分别作为甲状腺癌组与癌旁组。采用实时荧光定量PCR(real-time quantitative polymerase chain reaction, RT-qPCR)法检测甲状腺癌组织中SOX9 mRNA表达水平;免疫组织化学法检测SOX9蛋白表达。根据免疫组织化学染色结果中SOX9蛋白表达将甲状腺癌患者分为阳性表达组(67例)与阴性表达组 (31例),观察其与患者临床病理特征的关系。对所有甲状腺癌患者进行3年随访,应用Kap l an -Meier法分析甲状腺癌患者3年累积生存率;采用Log-rank检验对影响患者3年总体生存期相关单因素进行分析;COX比例风险回归模型分析影响患者预后的多因素。结果:甲状腺癌组SOX9 mRNA及蛋白阳性表达率均显著高于癌旁组织(P<0.05);SOX9表达与淋巴结转移、TNM分期、包膜浸润明显相关(P<0.05);Kaplan-Meier法分析显示SOX9阴性表达组患者无进展生存期(progression-free sur vival,PFS)、总生存期(overall sur vival,OS)均显著高于阳性表达组(P<0.05);Log-rank分析显示高TNM分期、有淋巴结转移、有包膜浸润及SOX9阳性表达甲状腺癌患者平均生存时间明显缩短(P<0.05);COX多因素分析显示SOX9表达与淋巴结转移是影响甲状腺癌患者预后的独立危险因素。结论:甲状腺癌组织中SOX9表达水平明显升高,并可参与甲状腺癌发生及发展过程,SOX9高表达与患者预后差有关,可作为判断甲状腺癌病情进展及患者预后的重要指标。

SOX9 expression in thyroid cancer and its relationship with clinicopathological features and prognosis

Abstract

Objective: To investigate the expression of sex-determining region Y-box protein 9 (SOX9) in thyroid cancer and its relationship with clinicopathological characteristics and prognosis of patients. Methods: We retrospectively analysed of 98 patients with thyroid cancer admitted to Nanyang Central Hospital from August 2015 to December 2017; thyroid cancer tissues and paracancerous tissues excised during radical operation were collected as thyroid cancer group and paracancerous group respectively. The expression of SOX9 was detected by real-time quantitative polymerase chain reaction (RT-qPCR) and the expression of SOX9 was detected by immunohistochemistry. According to the expression of SOX9 protein in immunohistochemical staining, patients with thyroid cancer were divided into a positive expression group (67 cases) and a negative expression group (31 cases). The relationship between SOX9 expression and clinicopathological characteristics of patients was observed. All patients with thyroid cancer were followed up for 3 years. Kaplan-Meier method was used to analyze the 3-year cumulative survival rate of patients with thyroid cancer; Log-rank test was used to analyze the single factors related to the overall 3-year survival of patients; COX proportional risk regression model was used to analyze the multiple factors affecting the prognosis of patients. Results: The expression of SOX9 mRNA and protein in thyroid cancer was significantly higher than that in adjacent tissues (P<0.05); the expression of SOX9 was significantly correlated with lymph node metastasis, TNM stage and capsular infiltration (P<0.05); Kaplan-Meier analysis showed that progression-free survival (PFS) and overall survival (OS) in SOX9 negative expression group were significantly higher than those in positive expression group (P<0.05); Log-rank analysis showed that the mean survival time of thyroid cancer patients with high TNM stage, lymph node metastasis, capsular infiltration and SOX9 positive expression was significantly shortened (P<0.05); COX multivariate analysis showed that SOX9 expression and lymph node metastasis were independent risk factors affecting the prognosis of thyroid cancer patients. Conclusion: The expression level of SOX9 in thyroid cancer tissues is significantly increased, and can participate in the occurrence and development of thyroid cancer. The high expression of SOX9 is related to poor prognosis of patients, and can be used as an important indicator to judge the progression of thyroid cancer and prognosis of patients.
